摘 要:目的 研究γ-干扰素(IFN-γ)、CC趋化因子配体1(CCL1)基因多态性与糖尿病并发肺结核(DM-PTB)易感性的关联。方法 选择焦作市第三人民医院2019年2月-2020年12月收治的140例DM-PTB患者为研究组,另选取同期诊治的106例单纯肺结核(PTB组)和112例单纯DM(DM组)为对照,采用限制性片段长度多态性聚合酶链反应(PCR-RFLP)检测IFN-γ、CCL1基因多态性及其与DM-PTB易感性的相关性。结果 研究组IFN-γrs+874位点AA基因型、CCL1 rs15291位点AA基因型频率均高于PTB组和DM组,CCL1 rs15291位点AG基因型低于PTB组和DM组(P<0.05);研究组IFN-γrs+874等位基因A频率高于PTB组和DM组(P<0.05);多因素Logistic回归分析结果显示,文化程度、IFN-γrs+874 AA基因型、CCL1 rs15291 AA和AG基因型均是DM-PTB的影响因素(P<0.05)。结论 IFN-γ+874位点的AA基因型以及CCL1 rs15291的AA基因型是DM-PTB发病的易感基因型,与患者文化程度同为DM-PTB影响因素。OBJECTIVE To observe the correlation between polymorphisms of interferon-γ(IFN-γ) and CC chemokine ligand 1(CCL1) genes and susceptibility to pulmonary tuberculosis complicated with diabetes mellitus(DM-PTB).METHODS A total of 140 DM-PTB patients who were treated in the Third People′s Hospital of Jiaozuo from Feb 2019 to Dec 2020 were assigned as the study group,meanwhile,106 patients with simple pulmonary tuberculosis were chosen as the PTB group,and 112 patients with simple DM were set as the DM group.The polymorphisms of IFN-γ and CCL1 genes were detected by polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLF),and their association with the susceptibility to DM-PTB was observed.RESULTS The frequencies of AA genotype at rs+874 locus of IFN-γ and AA genotype at rs15291 locus of CCL1 were significantly higher in the study group than in the PTB group and the DM group;the frequency of AG genotype at rs15291 locus of CCL1 was significantly lower in the study group than in the PTB group and the DM group(P<0.05).The frequency of allele A at rs+874 of IFN-γ was significantly higher in the study group than in the PTB group and the DM group(P<0.05).The result of multivariate logistic regression analysis showed that education level,IFN-γ rs+874 AA genotype and CCL1 rs15291 AA and AG genotypes were the influencing factors for DM-PTB(P<0.05).CONCLUSION The genotype AA at +874 locus of IFN-γ and AA genotype at rs15291 locus of CCL1 are the susceptible genotypes for DM-PTB,and the education level is also an influencing factor for DM-PTB.
