摘　　要：目的 探讨糖皮质激素诱导转录因子1(GLCCI1)基因多态性对糖皮质激素治疗下呼吸道感染临床疗效的影响。方法 选择2019年6月-2021年6月漯河市第三人民医院收治的下呼吸道感染患者为研究对象。治疗3 d时通过肺部影像学改变评估其疗效,将痊愈患者纳入研究组,好转/无效患者纳入对照组,每组41例。分析患者GLCCI1基因rs37973位点基因分布,记录治疗前、治疗3 d时第一秒用力呼气容积(FEV1)、第一秒用力呼气容积/用力肺活量(FEV1/FVC),检测C-反应蛋白(CRP)、降钙素原(PCT)、中性粒细胞/淋巴细胞比值(NLR)。结果 研究组GLCCI1基因rs37973位点GG基因型、G等位基因频率高于对照组(P<0.05);治疗前两组肺功能指标FEV1、FEV1/FVC以及血清学指标CRP、PCT和NLR水平比较无统计学差异;治疗后两组FEV1、FEV1/FVC均较治疗前升高,CRP、PCT和NLR水平均较治疗前下降(P<0.05),且GG组治疗后FEV1、FEV1/FVC水平高于AA/AG组,CRP、PCT和NLR水平低于AA/AG组(P<0.05);GG组肺啰音完全消失时间和感染完全控制时间较AA/AG组缩短(P<0.05),退热时间、住院天数比较差异无统计学意义。结论 GLCCI1基因rs37973位点多态性对糖皮质激素辅助治疗下呼吸道感染临床疗效有一定影响,其中携带GG型患者抗感染治疗效果更佳。OBJECTIVE To explore the influence of glucocorticoid-induced transcription factor 1(GLCCI1) gene polymorphism on clinical efficacy of glucocorticoids in treatment of lower respiratory tract infection.METHODS The patients with lower respiratory tract infection who were treated in Luohe Third People′s Hospital from Jun 2019 to Jun 2021 were recruited as the study objects.The clinical therapeutic effects were evaluated by lung imaging on the third day of treatment,the patients who were cured were assigned as the study group,and the patients who were improved or ineffective were assigned as the control group,with 41 cases in each group.The genotypes at rs37973 locus of GLCCI1 gene were observed,the forced expiratory volume in one second(FEV1) and the forced expiratory volume in one second/forced vital capacity(FEV1/FVC) were recorded before the treatment and on the third day of treatment,and the C-reactive protein(CRP),procalcitonin(PCT) and neutrophil/lymphocyte ratio(NLR) were detected.RESULTS The frequencies of GG genotype and G allele at rs37973 locus of GLCCI1 gene were significantly higher in the study group than in the control group(P<0.05).There were no significant differences in the lung function indexes FEV1,FEV1/FVC and serologic indexes CRP,PCT and NLR between the two groups before the treatment;the FEV1 and FEV1/FVC of the two groups were significantly higher after the treatment than before the treatment,while the levels of CRP,PCT and NLR were significantly lower after the treatment than before the treatment(P<0.05);the levels of FEV1 and FEV1/FVC of the GG group were significantly higher than those of the AA/AG group after the treatment,while the levels of CRP,PCT and NLR of the GG group were significantly lower than those of the AA/AG group(P<0.05).The time of complete disappearance of pulmonary rales and the time of complete control of infection were significantly shorter in the GG group than in the AA/AG group(P<0.05).There were no significant differences in the fever abatement time and length of
