摘　　要：目的 分析中枢神经系统(CNS)感染患者脑脊液中陷窝蛋白-1(Caveolin-1)、外泌体微小核糖核酸家族(Let-7b)、基质金属蛋白酶-9(MMP-9)及白细胞介素-1β(IL-1β)的表达水平及其临床意义。方法 选择2016年6月-2021年2月三门峡市中心医院收治的CNS感染患者51例为病例组,其中病毒性脑膜炎28例(VM组),结核性脑膜炎23例(TBM组);选择非CNS感染患者25例为对照组。检测脑脊液Caveolin-1、MMP-9和IL-1β和Let-7b表达。结果 病例组患者脑脊液Caveolin-1、MMP-9、Let-7b相对表达量及IL-1β高于对照组,比较差异有统计学意义(P<0.05);VM组患者脑脊液Let-7b相对表达量高于TBM组,Caveolin-1、MMP-9及IL-1β的含量低于TBM组,比较差异具有统计学意义(P<0.05);受试者工作特征曲线(ROC)分析结果显示,脑脊液Caveolin-1、Let-7b、MMP-9及IL-1β诊断TBM有较高的临床价值。结论 CNS感染患者脑脊液Caveolin-1、MMP-9及IL-1β、Let-7b的表达增高,且Let-7b诊断TBM效能较高。OBJECTIVE To explore the expression levels of caveolin-1(Caveolin-1),exosomal microRNA family(Let-7 b),matrix metalloproteinase-9(MMP-9) and interleukin-1β(IL-1β) in cerebrospinal fluid of patients with central nervous system(CNS) infection and analyze the clinical significance.METHODS A total of patients with CNS infection who were treated in Sanmenxia Central Hospital from Jun 2016 to Feb 2021 were assigned as the case group,including 28 cases of viral meningitis(the VM group) and 23 cases of tuberculous meningitis( the TBM group);25 patients who did not have CNS infection were chosen as the control group.The expression levels of Caveolin-1,MMP-9,IL-1β and Let-7 b in cerebrospinal fluid were detected.RESULTS The relative expression levels of cerebrospinal fluid Caveolin-1,MMP-9,Let-7 b and IL-1β of the case group were significantly higher than those of the control group(P<0.05).The relative expression level of cerebrospinal fluid Let-7 b of the VM group was significantly higher than that of the TBM group,while the levels of Caveolin-1,MMP-9 and IL-1β of the VM group were significantly lower than those of the TBM group(P<0.05).The result of receiver operating characteristic(ROC) curve analysis showed that the cerebrospinal fluid Caveolin-1,Let-7 b,MMP-9 and IL-1β had high clinical value in diagnosis of TBM.CONCLUSION The patients with CNS infection show the rise of expression levels of cerebrospinal fluid Caveolin-1,MMP-9,IL-1β and Let-7 b,and Let-7 b has high efficiency in diagnosis of TBM.
