摘 要:目的分析细胞间黏附分子-1(ICAM-1)基因多态性与肾功能衰竭患者尿路感染易感性的关联性。方法回顾性分析2018年1月-2020年12月建德市第一人民医院124例慢性肾功能衰竭(CRF)患者的临床资料,根据是否合并尿路感染分为尿路感染组52例及非尿路感染组72例,并纳入124名同期入院体检者作为对照组。比较三组基线资料及血清核因子-κB(NF-κB)、肿瘤坏死因子-α(TNF-α)、干扰素γ(IFN-γ)水平差异,并记录三组ICAM-1 K469E、G241R基因多态性分布。结果尿路感染组和非尿路感染组患者肾小球滤过率(GFR)均低于对照组(P<0.05);尿路感染组血清NF-κB、TNF-α、IFN-γ均高于非尿路感染组及对照组(P<0.05),非尿路感染组则高于对照组(P<0.05);Hardy-Weinberg平衡检验提示各基因频率符合遗传平衡法则,具有较好群体代表性;三组K469E位点基因型及等位基因分布差异有统计学意义(P<0.05),其中尿路感染组KK基因频率及K等位基因频率均高于非尿路感染组和对照组(P<0.05);相较于非尿路感染者,KK基因型患尿路感染相对风险度OR=2.015,K等位基因OR=1.739;三组ICAM-1基因G241R位点基因型及等位基因分布比较,差异均无统计学意义。结论ICAM-1基因K469E多态性与CRF患者尿路感染易感性相关,K等位基因可能是CRF并发尿路感染的易感基因。OBJECTIVE To analyze the association between intercellular adhesion molecule-1(ICAM-1)gene polymorphism and susceptibility of urinary tract infection in patients with renal failure.METHODS The clinical data of 124 patients with chronic renal failure(CRF)in the First People′s Hospital of Jiande from Jan.2018 to Dec.2020 were retrospectively analyzed.According to whether complicated with urinary tract infection,they were divided into 52 cases of urinary tract infection group and 72 cases of non-urinary tract infection group,and 124 people who were admitted to the hospital for physical examination during the same period were selected as the control group.The baseline data and levels of serum nuclear factor-κB(NF-κB),tumor necrosis factor-α(TNF-α)and interferonγ(IFN-γ)were compared among the three groups,and the distribution status of ICAM-1 K469 E and G241 R gene polymorphisms were recorded in the three groups.RESULTS The GFR of urinary tract infection group and non-urinary tract infection group was lower than that of control group(P<0.05).Serum NF-κB,TNF-αand IFN-γin urinary tract infection group were higher than those in non-urinary tract infection group and control group(P<0.05),and the levels of above three indicators in non-urinary tract infection group were higher than those in control group(P<0.05).Hardy-Weinberg equilibrium test showed that the frequencies of each gene were in line with the law of genetic equilibrium and had a good group representation.There were significant difference in the genotype and allele distribution at K469 E locus among the three groups(P<0.05),and the frequency of KK gene and K allele in the urinary tract infection group were higher than those in the non-urinary tract infection group and the control group(P<0.05).Compared with patients with non-urinary tract infection,the relative risk of KK genotype was 2.015,and that of K allele was 1.739.There were no significant differenc in the genotype and allele distribution of the G241 R locus of of ICAM-1 gene among the three g
