摘 要:目的研究体检人群丙型肝炎病毒(HCV)感染情况及免疫学分型。方法选择2018年2月-2021年4月在天津市第四中心医院体检者11553名,检测其血清中抗-HCV情况,统计健康体检人群HCV感染情况,并分析其影响因素。检测免疫功能指标,分析HCV感染不同基因型患者的免疫功能。结果11553名体检者中,HCV感染149例,感染率为1.29%;不同年龄、居住地、饮酒史以及文化程度者HCV感染率差异有统计学意义(P<0.05);149例HCV感染者基因型包括1a、1b、2a、2b和3a 5种,其中以1b型为主(46.98%)。不同基因型患者性别差异有统计学意义(P<0.05),1b和2a型中男性占比较高;HCV感染者CD_(3)^(+)、CD_(4)^(+)以及CD_(4)^(+)/CD_(8)^(+)低于健康人,CD_(8)^(+)高于健康人(P<0.05);不同基因型HCV患者CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)水平以及HCV-RNA差异有统计学意义(P<0.05);多因素Logistic回归分析显示居住地为农村、文化程度为初中以下是健康体检者感染HCV的影响因素(P<0.05)。结论健康体检者中HCV感染率达1.29%,呈现多基因分布特点,以1b型为主,居住地为农村、文化程度为初中以下是健康体检者感染HCV的影响因素;HCV感染者存在免疫功能降低现象,且不同基因型患者免疫功能降低幅度以及病毒载量均不同。OBJECTIVE To study the status and immunological typing of hepatitis C virus(HCV)infection in healthy physical examination population.METHODS A total of 11553 individuals who underwent physical examination in Tianjin Fourth Central Hospital from Feb.2018 to Apr.2021 were selected,the anti-HCV in their serum was detected,the HCV infection status was counted,and its influencing factors were analyzed.The immune function indicators were detected,and immune function in HCV infected patients with different genotypes was analyzed.RESULTS Of the 11553 physical examination subjects,149 cases were infected with HCV,with an infection rate of 1.29%.There were statistically significant differences in the HCV infection rate among people with different ages,places of residence,drinking history and education levels(P<0.05).The genotypes of 149 HCV-infected patients included 1 a,1 b,2 a,2 b and 3 a,dominated by 1 b(46.98%).There was a significent difference in gender distribution among patients with different genotypes(P<0.05),and the proportion of males was higher in people infected with type 1 b and 2 a.The levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in HCV-infected patients were significantly lower than those in healthy people,and the CD8^(+)level was significantly higher than that in healthy people(P<0.05).There were significant differences in CD4^(+)and CD4^(+)/CD8^(+)levels and HCV-RNA among patients with different genotypes of HCV(P<0.05).Multivariate logistic regression analysis showed that living in rural areas and education level below junior high school were the influencing factors for HCV infection in healthy people(P<0.05).CONCLUSION The HCV infection rate among the healthy physical examination population was 1.29%,showing the characteristics of polygene distribution,mainly type 1 b.Living in rural areas and education level below junior high school were the influencing factors of HCV infection in healthy physical examination population.Individuals infected with HCV had decreased immune function,and patients with diffe
