多维锁定髓内钉在肱骨颈骨折中的疗效及对关节功能、日常生活能力的影响

摘　　要：目的:探究及观察多维锁定髓内钉在肱骨颈骨折中的疗效及对关节功能、日常生活能力的影响。方法:将2019年1月-2022年1月福州市长乐区医院收治的80例肱骨颈骨折患者根据随机数字表法分为对照组和观察组,各40例。对照组进行肱骨近端锁定钢板治疗,观察组则进行多维锁定髓内钉治疗。比较两组的总有效率、并发症发生情况、术后不同时间的关节功能(肩关节活动度及Constant肩关节功能评分)及手术前后的日常生活能力(Barthel指数)。结果:术后3个月及6个月,观察组的总有效率均高于对照组,并发症总发生率低于对照组,差异均有统计学意义(P<0.05);术后6、12及24周,观察组肩关节活动度及Constant肩关节功能评分均优于对照组,差异均有统计学意义(P<0.05);术前两组Barthel指数比较,差异无统计学意义(P>0.05),术后3个月及6个月观察组的Barthel指数均优于对照组,差异均有统计学意义(P<0.05)。结论:多维锁定髓内钉在肱骨颈骨折中的疗效较好,可显著改善患者的关节功能,提高日常生活能力,在肱骨颈骨折患者中的应用价值较高。Objective:To explore and observe the curative effect of multidimensional locking intramedullary nail for humeral neck fracture and the influence on joint function and activity of daily living.Method:A total of 80 patients with humeral neck fracture who were admitted to Fuzhou Changle District Hospital from January 2019 to January 2022 were divided into the control group and the observation group according to the randon number table,with 40 cases in each group.The control group was treated with proximal humeral locking plate,the observation group was treated with multidimensional locking intramedullary nail.The total effective rates,incidence of complications,joint function(shoulder joint range of motion and Constant shoulder function score)at different time after the operation,activity of daily living(Barthel index)before and after the operation were compared between the two groups.Result:3 months and 6 months after operation,the total effective rates of the observation group were higher than those of the control group,the complication rate was lower than that of the control group,the differences were statistically significant(P<0.05);6,12 and 24 weeks after operation,the shoulder joint range of motion and Constant shoulder function scores of the observation group were better than those of the control group,the differences were statistically significant(P<0.05),there was no significant difference in Barthel index between the two groups preoperatively(P>0.05),and the Barthel indexes in the observation group were better than those in the control group 3 months and 6 months after operation,the differences were statistically significant(P<0.05).Conclusion:Multidimensional locking intramedullary nail in the treatment of humerus neck fracture has good curative effect,can significantly improve the joint function of patients,improve the ability of daily living,the application value is higher in patients with fracture of humerus neck.
