摘  要:目的:分析小而密低密度脂蛋白胆固醇(sdLDL-C)、S-腺苷同型半胱氨酸(SAH)水平与冠心病患者冠状动脉狭窄程度的关系。方法:回顾性分析2017年7月-2020年6月苏州市相城人民医院收治的126例冠心病冠状动脉狭窄患者为观察组,同时选取同期体检健康者70例为对照组,均行sdLDL-C、SAH水平监测。依据美国心脏病学会推荐的冠状动脉狭窄程度(Gensini积分)评分标准将观察组分为轻度狭窄组(<5分,n=70)、中重度狭窄组(≥5分,n=56)。对比轻度狭窄组与中重度狭窄组sdLDL-C、SAH水平,并采用logistic回归分析法分析sdLDL-C、SAH水平与冠心病患者冠状动脉狭窄程度的关系。结果:观察组sdLDL-C、SAH水平均高于对照组(P<0.05)。轻度狭窄组与中重度狭窄组性别、年龄、体重指数、吸烟、饮酒、冠心病家族史对比,差异均无统计学意义(P>0.05)。中重度狭窄组总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、sdLDL-C、SAH水平均高于轻度狭窄组(P<0.05)。TC、TG、LDL-C、sdLDL-C、SAH水平均是冠心病冠状动脉中重度狭窄的危险因素(P<0.05)。结论:冠心病患者sdLDL-C、SAH水平呈现异常表达,可反映疾病严重程度,TC、TG、LDL-C、sdLDL-C、SAH水平均是冠心病中重度狭窄患者的危险因素,均与冠心病患者冠状动脉狭窄程度相关。Objective:To analyze the relationship between the levels of small dense low density lipoprotein cholesterol(sdLDL-C)and S-adenosine homocysteine(SAH)and the degree of coronary artery stenosis in patients with coronary heart disease.Method:A total of 126 patients with coronary artery stenosis treated in Suzhou Xiangcheng People’s Hospital from July 2017 to June 2020 were retrospectively analyzed as the observation group,and 70 healthy subjects in the same period were selected as the control group.The levels of sdLDL-C and SAH were monitored.According to the scoring standard of coronary artery stenosis(Gensini score)recommended by the American college of cardiology,the patients were divided into mild stenosis group(<5 points,n=70)and moderate and severe stenosis group(≥5 points,n=56).The levels of sdLDL-C and SAH in the mild stenosis group and the moderate and severe stenosis group were compared,the relationship between the levels of sdLDL-C and SAH and the degree of coronary artery stenosis in patients with coronary heart disease were analyzed by logistic regression analysis.Result:The levels of sdLDL-C and SAH in the observation group were higher than those in the control group(P<0.05).There were no significant differences in gender,age,body mass index,smoking,drinking and family history of coronary heart disease between the mild stenosis group and the moderate and severe stenosis group(P>0.05).The levels of TC,TG,LDL-C,sdLDL-C and SAH in moderate and severe stenosis group were higher than those in mild stenosis group(P<0.05).The levels of TC,TG,LDL-C,sdLDL-C and SAH were all risk factors for moderate and severe coronary artery stenosis(P<0.05).Conclusion:The levels of sdLDL-C and SAH in patients with coronary heart disease show abnormal expression,which can reflect the severity of the disease.The levels of TC,TG,LDL-C,sdLDL-C and SAH are risk factors for patients with moderate and severe coronary artery stenosis,which are related to the degree of coronary artery stenosis in patients with coronary heart disea
