摘　　要：目的:探讨并分析男性不育症患者半胱氨酸天冬氨酸蛋白酶-3(Caspase-3)、B细胞淋巴瘤因子2(Bcl-2)表达水平及其与精液常规参数、精子DNA完整性的关系。方法:选取2019年5月-2021年5月于上饶市妇幼保健院确诊的100例男性不育症患者作为观察组,根据世界卫生组织标准将患者分为少精组(n=58)和弱精组(n=42),另随机选取在本院进行健康体检的56例正常男性作为对照组。比较三组患者的Caspase-3、Bcl-2表达水平、精液常规参数(精液量、精液浓度、前向运动率和精子存活率)及精子DNA完整性[DNA碎片指数(DFI)]。采用Pearson相关性分析少精组与弱精组Caspase-3、Bcl-2表达水平与精液常规参数及精子DNA完整性的关系。结果:少精组及弱精组的Caspase-3、DFI水平均高于对照组,Bcl-2均低于对照组(P<0.05)。少精组Caspase-3、DFI水平均高于弱精组,Bcl-2水平低于弱精组(P<0.05)。少精组及弱精组的精液量、浓度、前向运动率及精子存活率均低于对照组(P<0.05)。少精组精液量、浓度、前向运动率及精子存活率均低于弱精组(P<0.05)。少精组与弱精组Caspase-3与精液量、浓度、前向运动率及精子存活率均呈负相关,与DFI呈正相关(P<0.05)。Bcl-2与精液量、浓度、前向运动率及精子存活率均呈正相关,与DFI呈负相关(P<0.05)。结论:男性不育症患者Caspase-3水平显著升高,Bcl-2显著降低,且其变化水平与精液常规参数、精子DNA完整性关系密切。Objective:To explore and analyze the expression levels of cysteine proteinase-3 (Caspase-3) and B-cell lymphoma/leukemia 2 (Bcl-2) and their relationship with semen routine parameters and sperm DNA integrity in male patients with infertility.Method:A total of 100 male patients with infertility admitted to Shangrao Maternal and Child Health Hospital from May 2019 to May 2021 were selected as observation group,according to World Health Organization criteria,they were divided into oligospermia group (n=58) and asthenospermia group (n=42),56 normal males undergoing physical examination were selected as control group.The expression levels of Caspase-3 and Bcl-2,semen routine parameters (semen volume,semen concentration,forward motility rate and sperm survival rate) and sperm DNA integrity (DFI) among the three groups were compared.The relationship between Caspase-3,Bcl-2 and semen routine parameters,sperm DNA integrity were analyzed by Pearson correlation analysis in oligospermia group and asthenospermia group.Result:The levels of Caspase-3 and DFI in the oligospermia group and asthenospermia group were higher than those in the control group,and the levels of Bcl-2in the oligospermia group and asthenospermia group were lower than those in the control group (P<0.05).The levels of Caspase-3 and DFI in the oligospermia group were higher than those in the asthenospermia group,the level of Bcl-2 in the oligospermia group was lower than that in the asthenospermia group (P<0.05).The semen volume,semen concentration,forward motility rate and sperm survival rate in the oligospermia group and asthenospermia group were lower than those in the control group (P<0.05).The semen volume,semen concentration,forward motility rate and sperm survival rate in the oligospermia group were lower than those in the asthenospermia group (P<0.05).Caspase-3 were negatively correlated with semen volume,semen concentration,forward motility rate and sperm survival rate,and positively correlated with DFI in the oligospermia group and asthenospermia g
