摘 要:目的:基于有限元分析3D重建技术在髋关节置换术中的应用价值。方法:回顾性分析2019年1月-2021年10月萍乡市人民医院收治的80例行髋关节置换术患者的临床资料,其中2019年1月-2020年6月43例采用全髋关节置换术治疗的患者为对照组,2020年7月-2021年10月37例采用基于有限元分析3D重建技术的髋关节置换术患者为观察组。比较两组手术时间、术中出血量、术后健侧和患侧下肢长度差、植骨愈合、假体位置、髋关节功能(Harris)评分及疗效。结果:观察组术中出血量少于对照组,手术时间短于对照组,术后健侧和患侧下肢长度差低于对照组(P<0.05)。观察组植骨愈合率和假体位置吻合率均高于对照组(P<0.05)。术后,两组Harris评分均高于术前,且观察组高于对照组(P<0.05)。结论:基于有限元分析3D重建技术在髋关节置换术应用过程中可有效分析假体-骨界面的应力特征,并利用有限元分析的应力特征对假体进行再优化设计,显著提升髋关节置换术的临床应用质量。Objective:To analyze the application value of 3D reconstruction technology based on finite element analysis in hip arthroplasty.Method:The clinical data of 80 patients with hip arthroplasty admitted to Pingxiang People’s Hospital from January 2019 to October 2021 were retrospectively analyzed,Among them,43 patients treated with total hip arthroplasty from January 2019 to June 2020 selected as the control group,37 patients with hip arthroplasty who underwent 3D reconstruction technology based on finite element analysis from July 2020 to October 2021 were selected as the observation group.The operation time,intraoperative blood loss,postoperative lower limb length difference between the healthy side and the affected side,bone graft healing,prosthesis position,hip function (Harris) score and efficacy were compared between the two groups.Result:The intraoperative blood loss in the observation group was less than that in the control group,the operation time in the observation group was shorter than that in the control group,and the length difference between the healthy side and the affected side in the observation group was lower than that in the control group (P<0.05).The bone graft healing rate and prosthesis position anastomosis rate in the observation group were higher than those in the control group (P<0.05).After operation,the Harris scores in both were higher than those before operation,and that in the observation group was higher than that in the control group (P<0.05).Conclusion:3D reconstruction technology based on finite element analysis can effectively analyze the stress characteristics of prosthesis bone interface in the application of hip arthroplasty,and re optimize the design of prosthesis by using the stress characteristics of finite element analysis,which can significantly improve the clinical application quality of hip arthroplasty.
