龙胆泻肝胶囊辅助治疗分泌性中耳炎的效果及对中耳积液中TNF-α、IL-8、TGF-β1和TGF-β2水平的影响

摘　　要：目的:探讨龙胆泻肝胶囊辅助治疗分泌性中耳炎(OME)的效果及对中耳积液中肿瘤坏死因子-α(TNF-α)、白介素-8(IL-8)、转化生长因子β1(TGF-β1)和转化生长因子β2(TGF-β2)水平的影响。方法:选取佳木斯东风区人民医院耳鼻咽喉科2019年1月-2022年1月收治的OME患者102例,按照随机数字表法将其分为对照组与观察组,每组51例。对照组采用鼓膜穿刺+西药治疗,观察组在对照组的基础上加用龙胆泻肝胶囊辅助治疗。比较两组疗效、不同频率的纯音听阈均值、中耳积液的炎症介质(TNF-α、IL-8、TGF-β1、TGF-β2)水平及并发症。结果:观察组总有效率为92.16%,高于对照组的76.47%(P<0.05)。治疗1个月后,两组频率500 Hz、1 000 Hz、2 000 Hz的纯音听阈均值均较治疗前降低,且观察组均低于对照组(P<0.05)。治疗1个月后,两组中耳积液的TNF-α、IL-8、TGF-β1、TGF-β2水平均较治疗前降低,且观察组均低于对照组(P<0.05)。观察组并发症发生率(17.65%)与对照组(13.73%)比较,差异无统计学意义(P>0.05)。结论:龙胆泻肝胶囊辅助治疗OME能提高疗效,改善中耳功能与听力水平,其机制可能与降低中耳积液的TNF-α、IL-8、TGF-β1和TGF-β2水平有关。Objective:To investigate the effect of Longdan Xiegan Capsules on the adjuvant treatment of otitis media with effusion (OME) and its effect on the levels of tumor necrosis factor-α (TNF-α),interleukin-8 (IL-8),transforming growth factor β1 (TGF-β1) and transforming growth factor β1 (TGF-β2) in middle ear effusion.Method:A total of 102 OME patients admitted to the Department of Otolaryngology,Dongfeng District People’s Hospital of Jiamusi from January 2019 to January 2022 were selected.They were divided into control group and observation group according to the random number table method,with 51 cases in each group.The control group was treated with tympanocentesis + western medicine,and the observation group was supplemented with Longdan Xiegan Capsules on the basis of the control group.The curative effect,pure tone average of different frequencies,levels of inflammatory mediators of ear middle effusion (TNF-α,IL-8,TGF-β1,TGF-β2) and complications between the two groups were compared.Result:The total effective rate of the observation group was 92.16%,higher than 76.47% of the control group (P<0.05).After 1 month of treatment,the pure tone average at frequencies of 500 Hz,1 000 Hz and 2 000 Hz in the two groups were decreased compared with before treatment,and those in the observation group were lower than those in the control group (P<0.05).After 1 month of treatment,the levels of TNF-α,IL-8,TGF-β1 and TGF-β2 in middle ear effusion of the two groups were decreased compared with before treatment,and those in the observation group were lower than those in the control group (P<0.05).The complication rate in the observation group (17.65%) compared with the control group (13.73%),the difference was not statistically significant (P>0.05).Conclusion:Longdan Xiegan Capsules can improve the curative effect,improve the middle ear function and hearing level in the adjuvant treatment of OME,and the mechanism may be related to reducing the levels of TNF-α,IL-8,TGF-β1 and TGF-β2 in middle ear effusion.
