CK-MB活性测定与质量测定对小儿病毒性肠炎心肌损伤的评估分析

摘　　要：目的:对比分析肌酸激酶同工酶-MB(CK-MB)活性测定法与质量测定法分别对小儿病毒性肠炎心肌损伤的判定价值。方法:回顾性分析2021年7-12月赣州市妇幼保健院收治的50例发生心肌损伤的病毒性肠炎患儿的临床资料作为心肌损伤组,另选取同期该院收治的50例未发生心肌损伤病毒性肠炎患儿的临床资料作为肠炎组。查阅并记录两组患儿的临床资料及CK-MB检查资料,分析CK-MB活性测定与质量测定在小儿病毒性肠炎中的检验结果。结果:心肌损伤组CK-MB活性测定和质量测定均高于肠炎组,差异均有统计学意义(P<0.05);绘制ROC曲线,结果显示,CK-MB活性测定与质量测定诊断小儿病毒性肠炎心肌损伤的AUC均>0.7,诊断效能较好;CK-MB质量测定对小儿病毒性肠炎心肌损伤诊断效能高于CK-MB活性测定,差异有统计学意义(Z=2.163,P=0.031)。结论:CK-MB质量测定在小儿病毒性肠炎心肌损伤中具有较高的诊断价值,可更好指导临床小儿病毒性肠炎心肌损伤的诊断及治疗。Objective:To compare and analyze the determination value of creatine kinase-MB (CK-MB) activity determination and quality determination in myocardial injury in children with viral enteritis.Method:The clinical data of 50 children with viral enteritis who had myocardial injury treated in Ganzhou Maternal and Child Health Care Hospital from July to December 2021 were retrospectively analyzed as the myocardial injury group,and the clinical data of 50 children with viral enteritis who without myocardial injury treated in our hospital in the same period were selected as the enteritis group.The clinical data and CK-MB examination data of children in the two groups were consulted and recorded,and the test results of CK-MB activity determination and quality determination in children with viral enteritis were analyzed.Result:The activity and quality determination of CK-MB in myocardial injury group were significantly higher than those in enteritis group,the differences were statistically significant (P<0.05);the ROC curve results showed that,the AUC of CK-MB activity determination and quality determination in the diagnosis of myocardial injury in children with viral enteritis were >0.7,the diagnostic efficiency was better.The diagnostic efficiency of CK-MB quality determination for myocardial injury in children with viral enteritis was higher than that of CK-MB activity determination,the difference was statistically significant (Z=2.163,P=0.031).Conclusion:The determination of CK-MB quality has higher diagnostic value in myocardial injury of children with viral enteritis,and can better guide the clinical diagnosis and treatment of myocardial injury of children with viral enteritis.
