利拉鲁肽注射液联合精蛋白锌重组赖脯胰岛素混合注射液和二甲双胍治疗肥胖型T2DM患者的效果分析

摘　　要：目的:探讨利拉鲁肽注射液联合精蛋白锌重组赖脯胰岛素混合注射液(50R)(商品名:优泌乐50)和二甲双胍治疗肥胖型2型糖尿病(T2DM)患者的效果。方法:选取2019年6月-2021年12月黑龙江省中医药科学院收治的96例肥胖型T2DM患者,按随机数字表法将患者分为对照组和观察组,各48例。所有患者均应用二甲双胍,对照组再给予优泌乐50治疗,观察组在对照组基础上增加利拉鲁肽。对比两组血糖相关指标、血脂水平、微炎症状态、氧化应激状态、不良反应。结果:治疗4个月,观察组空腹血糖(FPG)、糖化血红蛋白(HbA1c)、胰岛素抵抗指数(HOMA-IR)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、人急性时相血清淀粉样蛋白A(ASAA)、肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)均低于对照组(P<0.05)。治疗4个月,观察组过氧化氢酶(CAT)、丙二醛(MDA)均低于对照组,超氧化物歧化酶(SOD)高于对照组(P<0.05)。观察组的不良反应发生率为18.75%,高于对照组的12.50%,但差异无统计学意义(P>0.05)。结论:在二甲双胍与优泌乐50的基础上增加利拉鲁肽有助于改善肥胖型T2DM患者的血糖和血脂代谢,减轻患者机体的微炎症和氧化应激状态,且具有不增加不良反应的优点。Objective:To investigate the effect of Liraglutide Injection combined with Protamine Zinc Recombinant Insulin Lispro Mixed Injection(50R)(trade name:Youmille 50)and Metformin in the treatment of obese patients with type 2 diabetes mellitus(T2DM).Method:A total of 96 obese patients with T2DM who were admitted to Heilongjiang College of Traditional Chinese Medicine from June 2019 to December 2021 were selected,they were divided into the control group and the observation group according to the random number table method,with 48 cases in each group.All patients were given Metformin,the control group was given Youmille 50 treatment,and the observation group was given Liraglutide on the basis of the control group.The blood glucose-related indicators,blood lipid levels,micro-inflammatory state,oxidative stress state and adverse reactions were compared between the two groups.Result:After 4 months of treatment,the fasting blood glucose(FPG),glycosylated hemoglobin(HbA1c),insulin resistance index(HOMA-IR),total cholesterol(TC),triacylglycerol(TG),low density lipoprotein cholesterol(LDL-C),serum amyloid A(ASAA),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)in the observation group were lower than those in the control group(P<0.05).After 4 months of treatment,the catalase(CAT)and malondialdehyde(MDA)in the observation group were lower than those in the control group,the superoxide dismutase(SOD)in the observation group was higher than that in the control group(P<0.05).The incidence of adverse reactions in the observation group was 18.75%,which was higher than 12.50%in the control group,but the difference was not statistically significant(P>0.05).Conclusion:The addition of Liraglutide on the basis of Metformin and Youmille 50 helps to improve blood glucose and blood lipid metabolism in obese T2DM patients,reduce the micro-inflammation and oxidative stress state of the patient’s body,and has the advantage of not increasing adverse reactions.
