血吸虫肝纤维化老年患者腹腔镜胆囊切除术分析

摘　　要：目的分析血吸虫肝纤维化老年患者的腹腔镜胆囊切除术特征,并评估手术可行性及并发症等风险.方法选取2018年1月至2020年8月胆囊结石患者(>60岁),按照是否合并血吸虫性肝纤维化分为2组,血吸虫性肝纤维化131例(观察组)、无血吸虫性肝纤维化1,265例(对照组).分析腹腔镜胆囊切除术在血吸虫肝纤维化老年胆囊结石患者中的可行性,比较两组手术时间、出血量、住院时间、引流液量、术后并发症及肝功能.结果观察组手术时间、住院时间、抗生素使用时间为(49.23±24.71)min、(8.67±4.71)d、(5.03±3.95)d,均高于对照组(40.07±26.56)min、(5.92±2.54)d、(2.96±2.44)d,差异有统计学意义(P<0.05).两组出血量、引流管放置时间、引流液量、并发症发生率差异无统计学意义(P>0.05).观察组谷氨酰转移酶比对照组更高(P<0.05).结论血吸虫肝纤维化老年患者胆囊切除术是安全的.Objective To analyze the charateristis of laparoscopic chlecystectomy in elderly patients with schistosomiasis liver fibrosis,and to evaluate the fesibiliy of the operation and the risks of complications.Methods Patients with gllbadder stone(>60 years)adnitted to our hospital as the research object from January 2018 to August 2020 were selected and divided into 2 groups according to whether they were associated with schistosomiasis liver fibrosis:131 cases of schisomiasiss liver fibrosis(observation group)and 1,265 cases of normal liver fibrosis without shisosomis(control group)-The feasibility of laparoscopic cholecystectomy in elderly patients with schistosomiasis liver fibrosis with gllbadder stones were analyzed.The operation time blood loss,hospitaliz ation time,drainage volume,postoperative complications,etc.were compared.The liver function was analyzed in the two groups of patients.Results The operaion time,hospitalization time,and antibiotic use time of the observation group were(49.23±24.71)min,(8.67±4.71)d,and(5.03±3.95)d,which were all higher than those of the control group(40.07±26.56)min,(5.92±2.54)d,(2.96±244)d,and the diference was stitically signifcant(P<0.05).There were no significant diferences between the observation group and the normal group in intaoperative blood loss,drninage tube placement time,drainage fluid volume,and complication rate between the two groups(P>0.05).Analysis of liver function found that compared with non-liver fibrosis patents,patients with liver fibrosis have higher transglutaminase.Conclusion Cholecystectomy in elderly patients with shitsomis liver fibrosis is safe.
