摘  要:目的评价血清瘦素水平与妊娠期糖尿病(gestational diabetes mellitus,GDM)发病风险的关系。方法检索PubMed、Web of Science、EMBASE、Medline(OVID)数据库,检索时限自建库至2021年4月30日所有比较GDM和非GDM血清瘦素的病例-对照研究或队列研究。用Revman5.4和Stata16.0统计学软件进行Meta分析。结果共纳入14篇文章,包含1726例孕妇。GDM组与非GDM组血清瘦素比较差异有统计学意义(SMD=0.32,95%CI:0.15~0.49,P<0.001)。在孕中期(SMD=0.17,95%CI:0.04~0.30)和孕晚期(SMD=0.54,95%CI:0.16~0.91)GDM组血清瘦素水平均高于非GDM组,孕晚期更明显。在不同亚组中,亚洲地区(P=0.03)和欧洲地区(P<0.001)中GDM组与非GDM组瘦素水平比较,差异有统计学意义,而北美洲地区差异无统计学意义(P=0.42),其余亚组分析比较,差异均有统计学意义(P均<0.05)。结论GDM患者较正常孕妇体内存在较高水平瘦素,通过不同亚组分析,发现妊娠期瘦素水平与GDM发生风险呈正相关。Objective To evaluate the relationship between serum leptin level and risk of gestational diabetes mellitus.Methods We detailedly searched the PubMed,Web of Science,EMBASE,Medline(OVID)databases,all case-control studies or cohort studies comparing serum leptin of GDM and non-GDM were searched from self-built database to April 30th,2021.Meta-analysis was performed by Revman5.4 and Stata16.0statistical software.Results A total of 14 articles were included,including 1726 pregnant women.The analysis showed that there was a significant difference between serum leptin in GDM group and that in non-GDM group(SMD=0.32,95%CI:0.15-0.49,P<0.001).In the second trimester(SMD=0.17,95%CI:0.04-0.30)and the third trimester(SMD=0.54,95%CI:0.16-0.91),the serum leptin level in GDM group was higher than that in non-GDM group,especially in the third trimester.Among the different subgroups,there were statistically significant differences in leptin levels between GDM group and non-GDM group in Asia(P=0.03)and Europe(P<0.001),but there was no statistical difference in North America(P=0.42),and the other subgroups were statistically significant(P<0.05).Conclusion There is a higher level of leptin in GDM patients than in normal pregnant women.Through the analysis of different subgroups,it is found that the level of leptin during pregnancy is positively correlated with the risk of GDM.
