北京市不同医疗机构突发公共卫生事件管理模式和应对能力分析

摘　　要：目的通过分析北京市不同医疗机构新发重大传染病管理机构设置情况,了解各级各类医疗机构突发公共卫生事件管理模式和应对能力。方法采用χ~2检验,从5个方面对比不同医疗机构新发重大传染病管理机构设置占比和应对能力。结果突发公共卫生事件领导管理架构和流程运转机构设置在一级、二级与三级,公立与民营,城区与郊区,综合与专科,西医与中医医疗机构,差异均无统计学意义。二级医疗机构发热门诊或发热筛查哨点的设置比例明显低于三级和一级医疗机构,公立医疗机构发热门诊或发热筛查哨点的设置比例明显高于民营医疗机构,城区医疗机构有预检分诊和筛查流程的比例明显高于郊区医疗机构。结论北京市各级各类医疗机构虽然逐步形成了突发公共卫生事件管理模式,但应对能力有待提升。Objective To acquire the management mode and response capacity of public health emergencies in medical institutions by comparatively analyzing the establishment of management institutions for emerging major infectious diseases in different medical institutions of Beijing.Methods Chi-square test was used to compare the proportion and response capacity of new major infectious disease management institutions in different medical institutions from five different dimensions.Results The structure of leadership and management of public health emergencies and the institutional set-up for running the process did not show significant differences between primary,secondary and tertiary,public and private,u rban and suburban,general and specialist,and Western and Chinese medical institutions.The proportion of fever clinics or sentinel points for fever screening in secondary medical institutions was significantly lower than that in tertiary and primary medical institutions.The proportion of fever clinics or sentinel points for fever screening in public medical institutions was significantly higher than that in private medical institutions.The proportion of pre-screening,triage and screening procedures in urban medical institutions was significantly higher than that in suburban areas.Conclusion Various medical institutions at all levels in Beijing have gradually formed a management model for public health emergencies,but the coping capabilities needed to be improved.
