前瞻性心理护理在持续性姿势-感知性头晕患者前庭康复训练中的应用

摘　　要：目的探讨心理重塑等前瞻性心理护理在持续性姿势-感知性头晕患者(PPPD)前庭康复训练过程中的应用效果。方法选择52例诊断为PPPD患者,随机分为观察组和对照组,每组26例,2组年龄及性别无明显差异。对照组给予药物治疗、前庭康复治疗,观察组在对照组治疗的基础上增加前瞻性心理护理,采用眩晕残障程度评定量表(DHI)、汉密尔顿焦虑量表(HAMA)、Berg平衡量表(BBS)跟踪分析。结果PPPD患者有不同的心理状态,前瞻性心理护理及相应的健康教育,分别对观察组及对照组患者治疗前、1周后、2周后、1个月后进行量表评分,2组年龄、性别差异均无统计学意义(P>0.05)。治疗1个月后,对照组DHI总分(54.57±1.37)分;HAMA分析为(27.21±2.78)分;Berg分析(41.29±2.26)分。观察组DHI总分(21.93±1.97)分;HAMA分析为(21.71±2.52)分;Berg分析(52.85±2.47)分,2组比较差异均有统计学意义(P<0.05)。结论前瞻性心理护理应用于PPPD患者前庭康复训练过程中,对其行心理重塑可更有效改善焦虑情绪,更快缓解眩晕症状,提高患者对PPPD的认知水平,帮助其早日康复。Objective To explore the effect of psychological remodeling method on the application of pro⁃spective psychological nursing in the vestibular rehabilitation training of patients with persistent postural-perceptu⁃al dizziness(PPPD).Methods Fifty-two patients diagnosed with PPPD were selected and randomly divided into an observation group and a control group,26 cases in each group.There was no significant difference in age and gender between the two groups.The control group was given drug treatment and vestibular rehabilitation treatment,while the observation group was given prospective psychological care on the basis of the treatment of the control group.tracking analysis.Results PPPD patients had different psychological states,prospective psychological care and corresponding health education,the observation group and the control group were scored by the scale be⁃fore,1 week,2 weeks,and 1 month after treatment.There was no statistically significant difference between the two groups(P>0.05).After 1 month of treatment,the total DHI score in the control group was 54.57±1.37;the HAMA analysis was 27.21±2.78;the Berg analysis was 41.29±2.26.The total DHI score of the observation group was 21.93±1.97,the HAMA analysis was 21.71±2.52,and the Berg analysis was 52.85±2.47,and there were signif⁃icant differences between the two groups(P<0.05).Conclusion Prospective psychological nursing is applied to the vestibular rehabilitation training of patients with PPPD,and psychological remodeling can more effectively im⁃prove anxiety,relieve vertigo symptoms faster,improve patients'cognitive level of PPPD,and help them recover sooner.
