中药联合司库奇尤单抗对斑块型银屑病患者因子白细胞p38MAPK活化水平及血清炎症因子ACTH、COR、GH、PRL的影响研究

摘　　要：目的 研究中药联合司库奇尤单抗对斑块型银屑病患者因子白细胞p38MAPK活化水平及血清炎症因子促肾上腺皮质激素(adrenocorticotropic hormone, ACTH)、皮质醇(cortisol, COR)、生长激素(growth hormone, GH)、泌乳素(prolactin, PRL)的影响。方法 将2020年1月—2020年6月医院诊治的斑块型银屑病患者72例作为研究对象,按照随机数字表法分为两组,对照组36例采用司库奇尤单抗治疗,观察组36例在对照组基础上采用中药柔肝和络饮治疗。比较两组的疗效,中医症状积分,白细胞p38丝裂原活化蛋白激酶(p38 mitogen-activated protein kinase, p38MAPK)活化水平,血清炎症因子ACTH、COR、GH、PRL水平。结果 观察组、对照组总有效率分别为94.4%(34/36)、86.1%(31/36),差异无统计学意义(P>0.05),观察组、对照组愈显率分别为55.6%(20/36)、30.6%(11/36),组间差异有统计学意义(P<0.05)。治疗前两组红斑、厚度、鳞屑方面评分差异均无统计学意义(P>0.05),治疗后两组的以上各项评分均有所降低,且观察组红斑、厚度、鳞屑方面评分均略低于对照组,但组间差异仍无统计学意义(P>0.05)。治疗前两组白细胞p38MAPK阳性细胞百分比评分、染色强度评分差异均无统计学意义(P>0.05),治疗后两组的阳性细胞百分比评分、染色强度评分均显著降低,并且观察组显著低于对照组(P<0.05)。治疗前两组血清炎症因子ACTH、COR、GH、PRL水平差异均无统计学意义(P>0.05),治疗后两组血清GH、PRL水平降低,血清ACTH、COR水平升高,并且观察组较对照组改善更为明显(P<0.05)。结论 中药联合司库奇尤单抗对斑块型银屑病患者治疗效果显著,能够改善白细胞p38MAPK活化水平及血清炎症因子ACTH、COR、GH、PRL水平,有利于控制疾病进展。Objective To study the effect of traditional Chinese medicine combined with secukinumab on the activation level of leukocyte p38 MAPK and the levels of serum inflammatory factors adrenocorticotropic hormone(ACTH),cortisol(COR),growth hormone(GH)and prolactin(PRL)in patients with plaque psoriasis.Methods A total of 72 patients with plaque psoriasis diagnosed and treated in the hospital from January to June of 2020 were taken as the research objects.They were divided into two groups according to the random number table method,36 cases in the control group were treated with skuccinoumab,and 36 cases in the observation group were treated with Chinese medicine Rougan Heluo Decoction(柔肝和络饮)on the basis of the control group.The curative effects,TCM symptoms score,activation level of white blood cell p38 MAPK,the levels serum of inflammatory factors ACTH,COR,GH and PRL of two groups were compared.Results The total effective rates of the observation group and control group were 94.4%(34/36)and 86.1%(31/36),respectively,and the difference was not statistically significant(P>0.05).The recovery rates of the observation group and control group were 55.6%(20/36)and 30.6%(11/36),respectively and the difference between two groups was statistically significant(P<0.05).Before treatment,there was no significant difference in the scores of erythema,thickness or scale between two groups(P>0.05).After treatment,the above scores of two groups were reduced,and the scores of erythema,thickness and scale of the observation group were slightly lower than those of the control group,and the differences between two groups were still not statistically significant(P>0.05).There was no statistically significant difference in the percentage scores of p38 MAPK positive cells and staining intensity between two groups before treatment(P>0.05).After treatment,the percentage of positive cells and staining intensity scores of two groups were significantly reduced,and those of the observation group were significantly lower than those of the co
