扶正化痰活血方治疗肾虚血瘀型复发性流产的效果分析

摘　　要：目的 分析扶正化痰活血方治疗肾虚血瘀型复发性流产的效果。方法 收集医院在2018年12月—2020年8月期间收治的肾虚血瘀型复发性流产患者106例,采用随机数字法分为对照组(地屈孕酮)和研究组(加用中医方剂扶正化痰活血方),对比两种方案治疗患者的临床效果。结果 研究组总有效率高于对照组,差异具有统计学意义(P<0.05)。两组治疗前D二聚体(D-dimer, D-D)、性激素结合球蛋白(sex hormone-binding globulin, SHBG)、狼疮样抗凝物质比值(lupus anticoagulant ratio, LARLAR)、炎症因子水平组间比较,差异无统计学意义(P>0.05)。研究组治疗后SHBG、白细胞介素-4(interleukin 4,IL-4)、白细胞介素-10(interleukin 10,IL-10)水平高于对照组,D-D、LAR、白细胞介素-2(IL-2)水平低于对照组(P<0.05)。研究组足月产率(64.15%,34/53)高于对照组(39.62%,21/53),流产率(18.87%,10/53)低于对照组(39.62%,21/53),早产率(16.98%,29/53)与对照组(20.75%,11/53)比较,差异无统计学意义(P>0.05)。结论 扶正化痰活血方治疗肾虚血瘀型复发性流产可有效减轻炎症反应,调节D-D、SHBG、LAR的表达水平,提高足月产率,减少流产率。Objective To analyze the effect of Fuzheng Huatan Huoxue Recipe(扶正化痰活血方)on recurrent abortion of kidney deficiency and blood stasis type.Methods A total of 106 patients with recurrent abortion of kidney deficiency and blood stasis type treated in the hospital from December 2018 to August 2020 were collected.They were randomly divided into control group(didroxyprogesterone)and study group(Fuzheng Huatan Huoxue Recipe),and the clinical effects of the two schemes were compared.Results The total effective rate of the study group was higher than that of the control group(P<0.05).There was no significant difference in the levels of D-Dimer(D-D),sex hormone binding globulin(SHBG),lupus like anticoagulant ratio(LAR)or inflammatory factors between the two groups before treatment(P>0.05).After treatment,the levels of SHBG,interleukin 4(IL-4)and interleukin 10(IL-10)in the study group were higher than those in the control group,and the levels of D-D,LAR and interleukin 2(IL-2)in the study group were lower than those in the control group(P<0.05).The full-term birth rate(64.15%,34/53)in the study group was higher than that(39.62%,21/53)in the control group.The abortion rate(18.87%,10/53)was lower than that(39.62%,21/53)in the control group and the preterm birth rate(16.98%,9/53)had no significant difference compared with that(20.75%,11/53)in the control group(P>0.05).Conclusion Fuzheng Huatan Huoxue Recipe can effectively reduce inflammatory reaction,regulate the expressions of D-D,SHBG and LAR,increase the full-term yield and reduce the abortion rate in the treatment of recurrent abortion with kidney deficiency and blood stasis type.
