“L”形自体肋软骨支架联合膨体聚四氟乙烯隆鼻术的临床观察

摘　　要：目的探讨“L”形自体肋软骨支架联合膨体聚四氟乙烯隆鼻术的临床应用效果。方法自2020年1月至2021年7月,河南中医药大学第一附属医院整形美容科对20例患者采用“L”形自体肋软骨支架联合膨体聚四氟乙烯行隆鼻术,均切取右侧第7肋软骨拐角的一段,搭建肋软骨支架,将已雕刻的膨体聚四氟乙烯假体置入已分离的腔隙,观察手术效果。结果20例患者获随访3~12个月,1例术后6个月诉鼻尖过高,局部麻醉下去除部分移植软骨后鼻尖形态改善;19例术后鼻外形改善明显,鼻延长明显,鼻尖形态良好,移植软骨无弯曲。结论“L”形自体肋软骨支架联合膨体聚四氟乙烯隆鼻术,可同时达到鼻尖增高及鼻延长的效果,具有良好的临床应用价值。Objective To explore the clinical efficacy of L-shaped autologous costal cartilage combined with expanded PTFE in augmentation rhinoplasty.Methods Twenty patients underwent augmentation rhinoplasty with L-shaped autologous costal cartilage combined with expanded PTFE in our department from January 2020 to July 2021 were retrospectively analyzed.The seventh costal cartilage on the right side was cut,and a segment of the corner of the cartilage was cut to build an L-shaped stent.The carved expanded PTFE prosthesis was placed into the space separated from the back of the nose.The efficacy of operation was observed.Results During the follow-up from 3 to 12 months,one patient complained that the nasal tip was too high and satisfied after removing part of the transplanted cartilage under local anesthesia.The nasal dorsum was natural and the nasal tip was in good shape.The improvement of nasal appearance was obvious in 19 patients.The nasal lengthening and nasal tip shape were good.Conclusion The L-shaped autologous costal cartilage combined with expanded PTFE for augmentation rhinoplasty can achieve the efficacy of nasal tip elevation and nasal lengthening at the same time.This method has good clinical application value.
