摘 要:目的 评价在急诊颅脑损伤患者凝血功能障碍及病情程度判断中血栓弹力图的应用效果及价值。方法 纳入平邑县人民医院2018年10月—2021年10月收治的80例急诊颅脑损伤患者为研究对象,行血栓弹力图检测,判断凝血综合指数,将80例患者分为高凝组(25例)、低凝组(25例)、正常组(30例)。以GCS评分再将患者分为3组,即重度组(20例)、中度组(20例)、轻度组(40例),均行PT、TT、APTT、FDP、D-D水平、血栓弹力图,对比血栓弹力图、凝血指标检测结果。结果 高凝组反应时间、凝固时间(5.15±1.18)、(1.45±0.70)min,低凝组(8.45±2.20)、(3.95±1.42)min,与正常组对比差异有统计学意义(P<0.05);高凝组、低凝组的血栓最大幅度、凝固角、凝血块强度,与正常组对比差异有统计学意义(P<0.05),高凝组FDP高于正常组,D-D高于低凝组,差异有统计学意义(P<0.05)。结论 血栓弹力图用于急诊颅脑损伤患者凝血功能障碍及病情程度判断中,效果显著。Objective To evaluate the application effect and value of thromboelastography in the judgment of coagulation dysfunction and disease severity in emergency patients with traumatic brain injury. Methods A total of 80 emergency patients with craniocerebral injury who were admitted to Pingyi County People′s Hospital from October 2018 to October 2021 were included as the research subjects. Thromboelastography was performed to determine the comprehensive coagulation index, and 80 patients were divided into the hypercoagulable group(25 case),hypocoagulable group(25 cases), normal group(30 cases). The patients were divided into 3 groups according to GCS score, namely severe group(20 cases), moderate group(20 cases) and mild group(40 cases). PT, TT, APTT, FDP, D-D levels, thromboelastography were performed andcompared the thromboelastography, coagulation index test results. Results The reaction time and coagulation time were(5.15±1.18) min,(1.45±0.70)min in the hypercoagulable group,(8.45±2.20) min,(3.95±1.42) min in the hypocoagulable group, and the differences were statistically significant compared with the normal group(P<0.05);the maximum thrombus amplitude,coagulation angle, and clot strength were significantly different between the hypercoagulable and hypocoagulable groups and the normal group(P<0.05), and the FDP hypercoagulable group was higher than the normal group, D-D was higher than the hypocoagulable group and the difference was statistically significant(P<0.05). Conclusion Thromboelastography is effective in judging the coagulation dysfunction and disease severity of emergency traumatic brain injury patients.
