多层螺旋CT泌尿系成像在输尿管梗阻性疾病中的临床诊断价值研究

摘　　要：目的分析多层螺旋CT泌尿系成像(CT urography,CTU)在输尿管梗阻性疾病中的诊断价值。方法选取2017年1月—2021年6月在徐州市铜山区中医院接受诊疗的疑似输尿管梗阻性疾病患者65例。患者均行CTU检查,然后将CTU诊断结果与手术或病理诊断结果进行比较。结果65例患者经手术或病理学诊断,61例患者确诊为阳性,包括泌尿结石患者34例、输尿管炎性狭窄患者14例、泌尿系肿瘤患者10例、输尿管畸形患者3例。经CTU诊断和手术或病理学诊断均为阳性的患者59例,其中泌尿系结石34例、输尿管炎性狭窄13例、泌尿系肿瘤10例、输尿管畸形2例;经CTU诊断为阳性而手术或病理学诊断为阴性的患者1例;经CTU诊断为为阴性而手术或病理学诊断为阳性的患者2例;经CTU诊断和手术或病理学诊断均为阴性的患者3例。诊断准确率为95.38%、灵敏度98.33%、特异度60.00%。结论CTU可显著提升输尿管梗阻性疾病的临床诊断准确率,有利于临床准确判断疾病情况,可为临床治疗提供可靠的参考。Objective To analyze the diagnostic value of multi-slice spiral CT urology(CTU)in ureteral obstructive diseases.Methods A total of65 patients with suspected ureteral obstructive disease who received diagnosis and treatment in Tongshan Hospital of Traditional Chinese Medicine from January 2017 to June 2021 were selected.All patients underwent CTU examination,and then the CTU diagnosis results were compared with the surgical or pathological diagnosis results.Results Sixty one of the 65 patients were confirmed positive by surgery or pathology,including 34 patients with urolithiasis,14 patients with inflammatory stricture of ureter,10 patients with tumor of urinary system,and 3patients with ureteral malformation.59 patients were diagnosed as positive by CTU,surgery or pathology,including 34 cases of urinary calculi,13 cases of ureteral inflammatory stenosis,10 cases of urinary tumors,and 2 cases of ureteral malformations.One patient was diagnosed as positive by CTU and negative by surgery or pathology.Two patients were diagnosed as negative by CTU and positive by surgery or pathology.Three patients were diagnosed as negative by CTU,surgery or pathology.The diagnostic accuracy was 95.38%,the sensitivity was98.33%,and the specificity was 60.00%.Conclusion CTU can significantly improve the accuracy of clinical diagnosis of ureteral obstructive diseases,which is conducive to accurate clinical diagnosis of diseases,and can provide reliable reference for clinical treatment.
