沙库巴曲缬沙坦治疗心房颤动合并心力衰竭的优势研究

摘　　要：目的探讨心房颤动合并心力衰竭患者采用沙库巴曲缬沙坦治疗的临床疗效及优势。方法选取2018年1月—2020年1于连云港市赣榆区人民医院接受治疗的心房颤动合并心力衰竭患者68例为研究对象。根据随机单双号抽签结果分为观察组(34例,单号)与对照组(34例,双号),对照组采取常规治疗,观察组在其基础上联合沙库巴曲缬沙坦治疗。比较两组临床治疗效果,检测对比两组患者治疗前后心功能指标(左室射血分数、左室收缩末期内径、左室舒张末期内径)及神经内分泌因子(血管紧张素、血浆N末端脑钠肽前体)变化。结果观察组患者通过沙库巴曲缬沙坦治疗,总有效率94.12%高于对照组,差异有统计学意义(χ^(2)=6.476,P<0.05)。观察组患者治疗后,左室射血分数高于对照组,左室收缩末期内径、舒张末期内径低于对照组,差异有统计学意义(P<0.05)。观察组患者治疗后神经内分泌因子指标均低于对照组,差异有统计学意义(P<0.05)。结论心房颤动合并心力衰竭患者通过采用沙库巴曲缬沙坦药物治疗具有良好临床疗效,能够有效改善患者心功能,提倡运用推广。Objective To investigate the clinical efficacy and advantages of sacubitril-valsartan in the treatment of patients with atrial fibrillation complicated with heart failure.Methods A total of 68 patients with atrial fibrillation and heart failure who were treated in Ganyu District People′s Hospital of Lianyungang City from January 2018 to January 2020 were selected as the research objects.According to the results of random odd and even numbers,they were divided into observation group(34 cases,odd number)and control group(34 cases,even number),the control group received conventional treatment,and the observation group was combined with sacubitril and valsartan.Comparison of clinical treatment effects between the two group.The cardiac function indexes(left ventricular ejection fraction,left ventricular end-systolic diameter,left ventricular end-diastolic diameter)and neuroendocrine factors(angiotensin,plasma N-terminal brain natriuretic peptide precursor)before and after treatment were detected and compared between the two groups.Results The patients in the observation group were treated with sacubitril and valsartan,and the total effective rate was 94.12%higher than that in the control group,and the difference was statistically significant(χ^(2)=6.476,P<0.05).After treatment,the left ventricular ejection fraction in the observation group was higher than that in the control group,and the left end-systolic diameter and end-diastolic diameter were lower than those in the control group,and the difference was statistically significant(P<0.05).The neuroendocrine factor indexes of the observation group after treatment were lower than those of the control group,and the difference was statistically significant(P<0.05).Conclusion The use of sacubitril and valsartan in the treatment of patients with atrial fibrillation and heart failure has a good clinical effect,which can effectively improve the cardiac function of patients,and its application and promotion are recommended.
