单双侧经皮穿刺椎体成形术治疗骨质疏松性压缩骨折对患者疼痛缓解程度及手术时间的影响

摘　　要：目的:探讨单双侧经皮穿刺椎体成形术(PVP)治疗骨质疏松性压缩骨折(OVCFs)对患者疼痛缓解程度(VAS评分)及手术时间的影响。方法:选取2020年3月至2022年3月收入遵义市第一人民医院的36例OVCFs患者作为观察对象。所有患者的手术均在局麻下进行,将能耐受双侧穿刺的患者分为对照组,将对穿刺所致疼痛耐受不佳的患者分为观察组,每组各18例。对照组接受双侧PVP手术治疗,观察组接受单侧PVP手术治疗,比较两组患者的术中各项指标、术后不同时段的腰背部疼痛程度。结果:观察组的手术时间短于对照组,术中出血量、X线曝光次数及骨水泥填充量少于对照组,差异均有统计学意义(P<0.05)。两组患者术前及术后3天、1个月、6个月的VAS评分相比,差异均无统计学意义(P>0.05)。结论:单双侧穿刺PVP手术用于OVCFs的治疗均可有效缓解椎体骨折所致的腰背部疼痛症状,改善患者的生活质量,但单侧PVP手术的用时更短,术中出血量、X线曝光次数以及骨水泥填充量更少,对于有适宜指征或无法耐受双侧椎弓根穿刺的患者,可根据具体情况酌情考虑给予单侧穿刺PVP治疗。Objective:To investigate the effects of unilateral and bilateral percutaneous vertebroplasty(PVP)in the treatment of osteoporotic compression fractures(OVCFs)on pain relief(VAS score)and operative time.Methods:36 patients with OVCFs admitted to the First People’s Hospital of Zunyi City from March 2020 to March 2022 were selected as the observation objects.All patients underwent surgery under local anesthesia.Patients who could tolerate bilateral puncture were divided into control group,and patients with poor tolerance to pain caused by puncture were divided into observation group,with 18 cases in each group.The control group received bilateral PVP surgery,and the observation group received unilateral PVP surgery.The intraoperative indexes and the degree of lower back pain in different postoperative periods were compared between the two groups.Results:The operation time of the observation group was shorter than that of the control group,and the amount of intraoperative blood loss,X-ray exposure times and bone cement filling volume were less than that of the control group,the differences were statistically significant(P<0.05).There was no significant difference in VAS scores before surgery,3 days,1 month and 6 months after surgery between 2 groups(P>0.05).Conclusion:Unilateral and bilateral PVP for OVCFs treatment can effectively relieve the symptoms of lumbar and back pain caused by vertebral fractures and improve the patients’quality of life.However,unilateral PVP surgery requires shorter time,less intraoperative blood loss,X-ray exposure times and bone cement filling volume.For patients with appropriate indications or who cannot tolerate bilateral pedicle puncture,Unilateral puncture PVP treatment may be considered a ccording to the specific circumstances.
