消瘀定痛散联合布洛芬缓释胶囊治疗膝关节滑膜炎疗效研究

摘　　要：目的:探讨消瘀定痛散联合布洛芬缓释胶囊对膝关节滑膜炎患者血清白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)、基质金属蛋白酶-3(MMP-3)的影响及临床疗效。方法:选取150例膝关节滑膜炎患者为研究对象,以随机数字表法分为西药组、中药组、联合组,每组50例。西药组以布洛芬缓释胶囊治疗,中药组以消瘀定痛散治疗,联合组以消瘀定痛散联合布洛芬缓释胶囊治疗。评估比较三组患者的症状评分、血液流变学、炎症因子、临床疗效、治疗安全性等指标。结果:治疗后观察组西安大略和麦克马斯特大学骨关节炎指数(WOMAC)评分低于治疗前,且联合组的评分低于西药组、中药组,差异有统计学意义(均P<0.05)。治疗后三组患者的全血黏度(WBV)、血小板聚集率(PAgT)、红细胞压积(HCT)、纤维蛋白原(Fib)均低于治疗前,且联合组低于西药组、中药组,差异有统计学意义(均P<0.05)。治疗后三组患者的IL-1β、TNF-α、MMP-3均低于治疗前,且联合组各项指标低于中药组、西药组,差异有统计学意义(均P<0.05)。联合组临床总有效率96.00%高于西药组74.00%、中药组78.00%,差异有统计学意义(P<0.05)。结论:消瘀定痛散联合布洛芬缓释胶囊对膝关节滑膜炎患者疗效较好,能改善患者症状评分,降低血液黏度,抑制IL-1β、TNF-α、MMP-3等炎症因子表达。Objective:To analyze effect and clinical efficacy of Xiaoyu Dingtong powder combined with ibuprofen sustained release capsule on serum interleukin-1β(IL-1β),tumor necrosis factorα(TNF-α)and matrix metalloproteinase-3(MMP-3)in patients with knee joint synovitis.Methods:150 patients with knee joint synovitis were selected as the study objects,divided into western medicine group,Chinese medicine group and combined group by random number table,with 50 cases each.The western medicine group were treated with ibuprofen sustained release capsule,the Chinese medicine group treated with Xiaoyu Dingtong powder,and the combined group treated with Xiaoyu Dingtong powder combined with ibuprofen sustained release capsule.The symptom score,hemorheology,inflammatory factors,clinical efficacy and treatment safety of the three groups were evaluated and compared.Results:After treatment,the scores of WOMAC in the observation group were lower than those before treatment and the scores of the combined group were lower than those in the western medicine group and the Chinese medicine group,difference statistically significant(all P<0.05).After treatment,the whole blood viscosity(WBV),platelet aggregation rate(PAgT),hematocrit(HCT)and fibrinogen(Fib)of the three groups were lower than those before treatment,and the indexes of the combined group were lower than those in the western medicine group and the Chinese medicine group,difference statistically significant(all P<0.05).After treatment,the levels of IL-1β,TNF-α,MMP-3 in the three groups were lower than those before treatment,and the indexes in the combination group were lower than those in the Chinese medicine group and the western medicine group,difference statistically significant(all P<0.05).The total clinical effective rate of combined patients was 96.00%,which was higher than the western medicine group 74.00%and the Chinese medicine group 78.00%,difference statistically significant(all P<0.05).The total effective rate of the combined group was 96.00%,which higher than that
