摘　　要：目的:探讨呼吸促进技术对脑卒中恢复期患者呼吸功能和步行能力的影响。方法:选取脑卒中恢复期患者30例,采用随机数表法分为对照组和观察组各15例。对照组予以常规康复治疗,观察组在常规康复治疗的基础上予以呼吸促进技术,包括深呼吸训练器和体外膈肌起搏疗法,每个疗程分别于治疗前、院内康复治疗2周后和家庭康复2周后评估患者呼吸功能、步行能力和日常生活活动能力等指标,观察3个疗程,共12周。结果:每个疗程治疗后,观察组患者呼吸功能(FVC、MEP、MIP和FEV1)、步行能力(6MWD、平均步速和最大步速)和日常生活活动能力(Barthel指数)等指标明显改善,组内比较具有显著性差异(P<0.05);3个疗程治疗后,两组间比较具有显著性差异(P<0.01)。结论:呼吸促进技术可改善脑卒中患者呼吸功能,提高下肢步行能力和日常生活活动能力,促进脑卒中患者早日回归家庭、回归社会。Objective:To explore the effect of facilitation techniques of breathing on respiratory function and walking ability for convalescent stroke patients.Method:A total of 30 stroke patients were randomly divided into control group and observation group with 15 cases each.Both groups received routine rehabilitation therapy,while the patients of the observation group accepted additional facilitation techniques of breathing,including deep breathing trainer and external diaphragm pacemaker.A course included 2 weeks treatment in hospital and 2 weeks at home.3 courses applied successively in 12 weeks.Their respiratory function,walking ability and activities of daily living were evaluated biweekly.Result:Respiratory function(FVC,MEP,MIP and FEV1),walking ability(6MWD,average stride speed and maximum stride speed)and activities of daily living(Barthel index)were evaluated once every two weeks before and after treatment.They were significantly improved time by time for both groups and the observation group improved more significantly(P<0.01).Conclusion:The facilitation techniques of breathing can improve the respiratory function lower limbs walking ability and activities of daily living of stroke patients,and promote them to return to family and society earlier.
