中西医结合外治法治疗早中期膝骨关节炎的疗效观察

摘　　要：目的:观察中西医结合外治法在早中期膝骨关节炎(KOA)的临床应用效果。方法:将106例早中期KOA患者随机分为观察组和对照组,每组各53例。观察组予中药熏洗联合氟比洛芬巴布膏贴敷,对照组予氟比洛芬巴布膏贴敷。比较两组治疗后临床疗效、视觉模拟评分(VAS)、膝关节活动度(ROM)、骨关节炎指数评分(WOMAC)及生活质量评分(SF-36)。结果:观察组总有效率为90.57%,高于对照组的75.47%,差异有统计学意义(P<0.05);治疗后,两组VAS、WOMAC评分较治疗前降低,且观察组低于对照组(P<0.05);两组ROM较治疗前提高,且观察组高于对照组(P<0.05);治疗结束3个月随访,两组SF-36各维度评分较治疗前提高,且观察组高于对照组(P<0.05)。结论:中药熏洗联合氟比洛芬巴布膏可缓解早中期KOA患者疼痛,改善膝关节功能,提高生活质量,是一种疗效确切、安全性较好的中西医结合外治法。Objective To observe the clinical effect of external treatment of integrated traditional Chinese and western medicine in early and middle stage knee osteoarthritis(KOA).Methods 106 patients with early and middle stage KOA were randomly divided into observation group and control group,with 53 cases in each group.The observation group was given traditional Chinese medicine fumigation combined with flurbiprofen cataplasm,and the control group was treated with flurbiprofen cataplasm.After treatment,the clinical efficacy,VAS score,ROM,osteoarthritis index score(WOMAC)and quality of life score(SF-36)were compared between the two groups.Results The clinical total effective rate of the observation group was 90.57%,and the control group was 75.47%,which the difference was statistically significant(P<0.05).After treatment,VAS and WOMAC scores in the two groups were lower than those before treatment,and the scores in the observation group were lower than those in the control group(P<0.05);The ROM of the two groups was higher than that before treatment,and the observation group was higher than the control group(P<0.05).Three months after treatment,the scores of SF-36 in the two groups were higher than those before treatment,and the scores in the observation group were higher than those in the control group(P<0.05).Conclusion Traditional Chinese medicine fumigation combined with flurbiprofen cataplasm can alleviate the pain of patients with early and middle stage KOA and improve their knee function and quality of life.It is an external treatment of integrated traditional Chinese and western medicine with definite curative effect and good safety.
