基于“阳化内风”理论探讨调肝法治疗回纹型风湿病

摘　　要：回纹型风湿病发病的核心病机为风邪,尤其是“内风”致痹的作用。肝“主筋束骨”,且为风木之脏,主升主动,肝阳偏亢化风,易伤筋动骨为病。在治疗中将风邪与肝藏象相结合,以叶天士“阳化内风”立论,强调“内风”致痹的发病特点。重视“调肝”的治疗大法,并结合五行生克制化理论,将滋肾、疏肝与健脾相结合;同时分析瘀血既是病因,又是病理产物,贯穿疾病发生的全程,配合活血化瘀疗法治疗该病,临床取得较为显著的疗效。The core pathogenesis of palindrome rheumatic(PR)is Wind evil factor,especially the role of"Nei Feng"(internal Wind)causing paralysis.The Liver"governs tendons and binds bones",and it is the Zand of Wind and Wood,which is related to Qi rising and movement.Hyperactivity of Liver Yang is easy turn into the Wind to damages tendons and bones and causes diseases.Therefore,in clinical treatment,Wind evil factor is often combined with Liver Zang and Xiang.In addition,based on Ye Tianshi′s"Yang Hua Nei Feng"theory,the pathogenic characteristics of"internal Wind"causing paralysis should be emphasized.At the same time,the treatment method of"regulating the Liver"should be paid attention to.Furthermore,by combining the theory of generation-inhibition in Five Elements Theory,nourishing the Kidney,soothing the Liver,and strengthening the Spleen should be used in combination.Last,it should be analyzed that Blood stasis is both the cause and the pathological product,which runs through the whole process of the disease.Therefore,by combining with the therapy of promoting Blood circulation and removing Blood stasis in clinical treatment,a significant clinical effect can be achieved.
