摘　　要：目的检索、评价并整合肿瘤患者输液港相关血栓预防及管理的相关证据,并对最佳证据进行总结。方法系统检索BMJ最佳临床实践、美国静脉输液护士协会(Infusion Nurses Society,INS)、Cochrane Library、JBI循证卫生保健国际合作中心图书馆、英国国家医疗保健优化研究所(National Institute for Health and Care Excellence,NICE)、加拿大安大略注册护士协会(Registered Nurses’Association of Ontario,RNAO)、Pub Med、Ovid、医脉通、中国知网、万方数据库、维普数据库、中国生物医学数据库等关于输液港相关血栓预防及管理的证据,包括指南、推荐实践、专家共识、系统评价等。检索时限为建库至2021年11月。结果共纳入7篇文献,其中指南4篇,系统评价1篇,专家共识2篇。从危险因素的评估、诊断与筛查、药物治疗、健康教育、拔除导管指征、质量管理6个方面汇总19条最佳证据。结论输液港相关血栓预防及管理的最佳证据可为临床医护人员实践提供参考,但应结合科室环境、患者实际情况应用最佳证据以降低输液港相关血栓,提高患者的生存质量。Objective To retrieve,evaluate and integrate the evidences on the prevention and management of venous thrombosis induced by implantable venous access port and to summarise the best evidence.Methods The websites or databases of BMJ Best Practice,the Infusion Nurses Society(INS),Cochrane Library,Joanna Briggs Institute Library,National Institute for Health and Care Excellence(NICE),Registered Nurses'Association of Ontario(RNAO),PubMed,Ovid,Medlive,CNKI,Wangfang Data,VIP database and Chinese Biomedical Literature Database(CBM)were searched from the inception to November 2021,for the purpose to retrieve the literatures including guidelines,recommended practices,statements of expert consensus and systematic reviews regarding venous thrombosis induced by implantable venous access port.Results A total of 7 papers were retrieved including 4 guidelines,1 systematic review,and 2 expert consensuses.Nineteen pieces of the best evidence were categorised into 6 topics:assessment of risk factors,diagnosis and screening,drug treatment,health education,indications for catheter removal,and quality management.Conclusions The best evidence for prevention and management of venous thrombosis induced by implantable venous access port can provide references for doctors and nurses,but it should be selected and applied accordingly to the actualities of the specific department and patient conditions so as to reduce the incidence rate of port-induced thrombosis and improve the quality of life of patients.
