摘 要:目的整合肾移植受者术后营养管理的最佳证据,为肾移植受者术后临床照护提供参考。方法计算机检索JBI循证护理数据库、国际指南协作网、美国国立指南库、CochraneLibrary、PubMed、OVID、Medline及中国知网、万方数据库等国内外指南网与数据库,获取关于肾移植受者营养管理相关指南、证据总结、系统评价、专家共识等。检索时限为2010年1月至2022年2月。由2名研究者对纳入的文献进行质量评价,并对证据进行提取、整合。结果本研究共纳入13篇文献,包括指南6篇、系统评价5篇、专家共识2篇。从肾移植受者营养风险筛查与评估、营养管理途径、能量及蛋白质摄入、微量营养素补充、饮食管理、吸烟与饮酒、食品安全7个方面,总结35条证据。结论总结的最佳证据可为肾移植受者术后营养管理提供循证参考。医护人员可根据肾移植受者自身条件、临床情景、文化背景及价值观以及证据应用利益相关者主观意愿、促进及阻碍因素等选择最佳证据,以形成科学、有循证依据的膳食营养管理方案。Objective To integrate the best evidence of postoperative nutritional management of kidney transplant recipients and provide reference for postoperative clinical care of kidney transplant recipients.Methods The JBI evidence-based nursing database,Guidelines International Network,Guidelines International Network,Cochrane library,PubMed,Ovid,MEDLINE,CNKI,Wanfang data and other domestic and foreign guideline networks and databases were searched by computer,and the relevant guidelines,evidence summary,systematic review,expert consensus on the management of kidney transplant recipients were obtained.The retrieval time limit is from January 2010 to February 2022.Extract evidence according to the principles and standards of document quality evaluation.Results A total of 13 literatures were included in this study,including 6 guidelines,5 systematic reviews,and 2 expert consensus.35 pieces of evidence were summarized from seven aspects:nutritional risk screening and evaluation,nutritional management approaches,energy and protein intake,micronutrient supplementation,dietary management,smoking and drinking,and food safety in kidney transplant recipients.Conclusions The best evidence summarized can provide evidence-based reference for postoperative nutritional management of kidney transplant recipients.Medical staff can select the best evidence according to the conditions of kidney transplant recipients,clinical situations,cultural backgrounds and values,as well as the subjective wishes,promotion and obstacles of stakeholders in the application of evidence,so as to form a scientific and evidence-based dietary nutrition management plan.
