持续硬膜外分娩镇痛联合自由体位对产妇阴道分娩的影响

摘　　要：目的 研究持续硬膜外分娩镇痛联合自由体位对产妇阴道分娩的影响。方法 选择120例产妇为研究对象,均行阴道分娩,依据随机数字表法将其分为对照组(60例,传统体位联合持续硬膜外麻醉镇痛)与研究组(60例,持续硬膜外麻醉镇痛联合自由体位),两组均观察至分娩结束。比较两组产妇自然分娩率与1 min新生儿阿氏(Apgar)评分、产程、分娩镇痛麻醉用药量与自控镇痛(PCA)用药量、阴道损伤以及不良反应发生情况。结果 研究组产妇的自然分娩率与新生儿1 min Apgar评分均高于对照组,第一产程与第二产程均短于对照组,麻醉用药量及PCA用药量均少于对照组,会阴损伤率及不良反应发生率均低于对照组,差异具有统计学意义(P<0.05)。结论 持续硬膜外分娩镇痛联合自由体位的方式有助于缓解阴道分娩产妇的疼痛,缩短产程,减少麻醉及镇痛用药量,对阴道损伤较轻,安全性较高。Objective To analyze the clinical effect of continuous epidural analgesia combined with free posture on vaginal delivery. Methods A total of 120 puerperae were selected as the research objects. Both groups underwent vaginal delivery.According to the random number table method, they were divided into the control group(60 cases, traditional position combined with epidural analgesia), and the study group(60 cases, free posture combined with epidural analgesia), both groups were observed until the end of labor. The natural delivery rate, 1 min Apgar score, delivery process, the amount of drugs used for labor analgesia and anesthesia, the amount of drugs used for patient controlled analgesia(PCA), vaginal injury and adverse reactions were compared between the two groups. Results The natural delivery rate and neonatal 1 min Apgar score in the study group were higher than those in the control group. The time of the first stage of labor and the second stage of labor in the study group were shorter than those in the control group, the dosage of anesthesia and PCA were less than those in the control group, and the perineal injury rate and adverse reaction rate were lower than those in the control group, the difference was statistically significant(P<0.05).Conclusion Continuous epidural analgesia combined with free posture can help relieve the pain of vaginal delivery women, shorten the labor process, reduce the amount of anesthesia and analgesia, and cause less vaginal damage and higher safety.
