中药冲洗加闭管灵灌注治疗大鼠皮下窦腔的作用及机理

摘  要:目的:探讨中药冲洗加闭管灵灌注治疗窦腔的作用及其机理。方法:皮下注射5%的酵母菌混悬液法建立大鼠皮下窦腔实验模型,分为模型组、复方黄柏液组、闭管灵高剂量组、闭管灵低剂量组,健康大鼠为对照组,每组8只。模型组选用生理盐水冲洗脓腔,复方黄柏液组用复方黄柏液冲洗脓腔,闭管灵高低剂量组采用复方黄柏液中药液冲洗后分别注入25%和20%闭管灵溶液。采用免疫组化法检测碱性成纤维细胞因子(FGF⁃2)和白细胞介素1(IL⁃1)的表达,采用HE染色检测脓腔组织的病变,培养脓腔腔道细菌检测细菌覆盖率。结果:对照组皮肤完整、无破损,模型组疮疡部位隆起、周围重度红肿或破溃,皮下窦腔实验模型制备成功。与对照组比较,模型组溃疡评分、组织FGF⁃2、IL⁃1表达明显增高;与模型组比较,复方黄柏液组、闭管灵高、低剂量组溃疡评分明显降低;与复方黄柏液组比较,闭管灵高、低剂量组溃疡评分明显降低,且呈给药时间依赖性降低趋势。闭管灵高剂量对疮疡周围正常的皮肤组织有腐蚀作用,低剂量对疮疡周围正常的皮肤组织未见损伤作用。结论:三组实验小鼠皮下窦腔均得到明显改善,尤其是闭管灵低剂量组对窦腔愈合及缓解疮疡周围正常皮肤组织损伤方面疗效显著。Objective:To explore the mechanism of Biguanling perfusion on the basis of traditional Chinese medicine in the treatment of sinus cavity of rats.Methods:The sinus cavity rat was established by subcutaneous injection of 5%yeast suspension,and rats were divided into the model group,Huangbai soultion group,high⁃and low⁃dose Biguanling groups,with 8 rats in each group.Eight normal rats were enrolled into the control group.For the model group,normal saline was used to irrigate the abscess cavity.For the Huangbai group,Huangbai soultion was used to wash the abscess cavity.For the high⁃and low⁃dose Biguanling groups,25%and 20%Biguanling soultion was aodped for irrigation of the abscess cavity,respectively.Immunohistochemistry was used for detect the expression of FGF⁃2 and IL⁃1,HE staining was used to observe the lesions of abscess tissue,and bacteria in the abscess cavity were cultured and bacterial coverage was detected.Results:The skin of the control group was intact and unbroken,while the sore site of the model group was uplifted,and the surround⁃ing area was severely red,swollen or ulcerated.The sinus cavity rat model was successfully pre⁃pared.Compared with that in the control group,the ulcer score,tissue FGF⁃2 and IL⁃1 expression in the model group,the high⁃and low⁃dose Biguanling groups were significantly increased,and showed a decreasing trend in a time⁃dependent manner.The high dose Biguanling had a corrosive effect on the normal skin tissue around the sore,and the low dose had no damage to the normal skin tissue around the sore.Conclusion:The subcutaneous sinus cavity of the three groups of experimental mice has been significantly improved,especially the low⁃dose Biguanling group has a significant effect on healing sinus cavity and relieving skin damage around the sore.
