血清白蛋白浓度、凝血酶原时间对新生儿坏死性小肠结肠炎Ⅲ期的诊断价值

摘　　要：目的:探讨血清白蛋白(Alb)浓度、凝血酶原时间(PT)对新生儿坏死性小肠结肠炎(NEC)Ⅲ期的诊断价值。方法:回顾性分析235例NEC患儿(Ⅱ期173例,Ⅲ期62例)及母亲资料,比较两组资料,进行相关统计学分析。结果:胎龄越小发病年龄越晚(P<0.01)。NEC严重程度的相关因素:母亲子痫前期(OR:4.16,95%CI:1.07~16.11)、晚期发病(OR:6.78,95%CI:1.22~37.52)、CRP(OR:1.03,95%CI:1.01~1.06)、Alb浓度(OR:0.59,95%CI:0.47~0.74)、PT(OR:1.71,95%CI:1.39~2.25)。Alb浓度低和PT延长对NECⅢ期有诊断价值(P<0.01)。结论:Alb浓度降低和PT延长是NEC严重程度的预警信号,有助于早期诊断重度NEC(Ⅲ期)。Objective:To explore the diagnostic value of serum albumin(Alb)concentration and prothrom⁃bin time(PT)for the stageⅢneonatal necrotizing enterocolitis(NEC).Methods:A retrospective analysis was performed on the clinical data of 235 neonates and their mothers with confirmed NECⅡ(173 cases)and NECⅢ(62 cases).The related factors to NEC were investigated by statistical analy⁃sis.Results:The onset age of the small gestational age infants was later than that of the large gesta⁃tional age infants(P<0.01).The related factors of the stageⅢNEC included the preeclampsia of mother(OR:4.16,95%CI:1.07⁃16.11),late onset age(OR:6.78,95%CI:1.22⁃37.52),increased CRP(OR:1.03,95%CI:1.01⁃1.06),decreased serum albumin concentration(OR:0.59,95%CI:0.47⁃0.74),and increased prothrombin time(PT)(OR:1.71,95%CI:1.39⁃2.25),respectively.Low serum albumin concentration and prolonged PT have diagnostic value for stageⅢNEC.Conclusion:Decreased serum Alb concentration and prolonged PT are early warning signs of the se⁃verity of NEC and are helpful for early diagnosis of severe NEC(stageⅢ).
