脑卒中偏瘫患者两侧肱动脉血压差异的比较及分析

摘　　要：目的:探讨脑卒中偏瘫患者偏瘫侧手臂与健侧手臂血压测量有无差异。方法:对103例脑卒中偏瘫患者测量两侧肱动脉血压,并将所测值进行比较,并对可能的影响因素进行单因素分析。结果:脑卒中偏瘫患者无论是左侧偏瘫还是右侧偏瘫,其健侧与患侧肱动脉的收缩压和舒张压相比,差异均有统计学意义(P<0.05),相关因素分析表明患侧肢体处于痉挛状态时,患侧肢体收缩压和舒张压均高于健侧肢体肱动脉血压,差异有统计学意义(P<0.05)。结论:脑卒中偏瘫患者偏瘫侧肢体肌张力处于痉挛这一特殊时期时,应该尽量测量健侧肢体血压,除此以外,测量任何一侧的上肢血压均可。Objective:To investigate the difference of blood pressure measurement between hemiplegic arm and healthy arm in stroke patients with hemiplegia.Methods:The blood pressure of brachial artery on both sides was measured in 103 patients with stroke hemiplegia,and the measured values were com⁃pared.The possible influencing factors were analyzed by single factor analysis.Results:There was significant difference in systolic and diastolic blood pressures of brachial artery between different sides of stroke patients with hemiplegia,whether left hemiplegia or right hemiplegia(P<0.05).The analy⁃sis on the related factors showed that there was a correlation between blood pressure and muscle ten⁃sion.Conclusion:When the muscle tension of hemiplegic limb of stroke patients is in the special peri⁃od of spasm,the blood pressure should be measured on the healthy limb,while when the spasm is re⁃moved,the blood pressure of upper limb on either side can be measured.
