Graf法联合Harcke法在发育性髋关节发育不良儿童诊断中的应用价值

摘　　要：目的:探讨Graf法联合Harcke法在发育性髋关节发育不良(DDH)儿童诊断中的应用价值。方法:选取2020年1月至2021年1月商丘市第三人民医院收治的疑似DDH婴幼儿150例(198髋)为研究对象,均接受Graf法和Harcke法检查。分析Graf法、Harcke法单独及联合对DDH的诊断价值,并以临床诊断结果作为金标准,采用Kappa检验分析Graf法、Harcke法单独及联合诊断DDH的一致性。结果:联合检查方法诊断DDH的灵敏度、准确度、阴性预测值均明显高于Graf法、Harcke法,差异具有统计学意义(P<0.05);Graf法诊断DDH的κ为0.790,Harcke法诊断DDH的κ为0.781,联合检查方法诊断DDH的κ为0.964,联合检查方法的一致性最高。结论:Graf法联合Harcke法能提高诊断DDH的灵敏度和准确度,且与临床诊断结果的一致性最高。Objective To explore the value of Graf method and Harcke method in the diagnosis of developmental dysplasia of the hip(DDH)in children.Methods A total of 150 infants(198 hips)with suspected DDH admitted to Shangqiu Third People's Hospital from January 2020 to January 2021 were selected as the research objects.All of them were examined by Graf and Harcke methods.The diagnostic value of Graf method and Harcke method alone and in combination for DDH was analyzed.The clinical diagnosis results were taken as the gold standard,and the consistency of Graf method and Harcke method alone and in combination for diagnosis of DDH was analyzed by Kappa method.Results The sensitivity,accuracy and negative predictive value of combined examination in the diagnosis of DDH were significantly higher than those of the Graf method and Harcke method,and the differences were statistically significant(P<0.05).The Kappa value of Graf method in the diagnosis of DDH was 0.790,the Kappa value of Harcke method in the diagnosis of DDH was 0.781,and the Kappa value of combined examination method in the diagnosis of DDH was 0.964.The consistency of combined examination method was the best.Conclusion Graf method combined with Harcke method can improve the sensitivity and accuracy of diagnosis of DDH,and the consistency with clinical diagnosis results is the best.
