简化血压测量方法在高血压筛查中的应用价值探讨

摘　　要：目的:探讨简化血压测量方法在高血压筛查中的应用价值。方法:选取2018年4月-2021年9月于沂南县皮肤病防治站5672名体检者作为研究对象,根据指南对所有体检者进行血压测量,以3次平均值作为诊断高血压的标准,根据自制的简化血压测量图对所有体检者血压进行分类诊断,分析简化血压测量表中各项目在高血压筛查中的应用效果。结果:5672名体检者中,经标准法检测确定高血压患者643例,筛出率为11.34%;经简化血压测量法筛出高血压患者637例,筛出率为11.23%,不同测量方式的筛出率比较,差异无统计学意义(P>0.05);简化血压测量方法中首次血压值(a)≥130/80 mmHg筛查高血压的灵敏度、特异性、假阳性率和假阴性率分别为95.96%、99.60%、0.40%和4.04%,二次血压测量比例为27.27%,与标准方法筛查高血压的Kappa值为0.959。结论:简化血压测量方法中以首次血压值(a)≥130/80 mmHg判定是否进行二次血压测量,可在保证与标准方法筛查结果一致性的同时,明显降低二次血压测量比例,缩短测量时间,提高效率,值得应用并予以推广。Objective:To explore the application value of simplified blood pressure measurement method in hypertension screening.Methods:A total of 5672 people for physical examination in Yinan County Skin Disease Prevention Station from April 2018 to September 2021 were selected as the study subjects,their blood pressure was measured according to the guidelines,and the average value of three measurements was calculated as the standard for hypertension diagnosis.According to the homemade simplified blood pressure measurement map,the blood pressure of all physical examination subjects was classified and diagnosed.The application effect of various items in the simplified blood pressure measurement table for hypertension screening was analyzed.Results:Among 5672 physical examination subjects,643 hypertensive patients were identified by the standard methods,and the screening rate was 11.34%;637 hypertensive patients were identified by simplified blood pressure measurement method,and the screening rate was 11.23%.There was no statistically significant difference in screening rate of different measurement methods(P>0.05);the sensitivity,specificity,false positive rate and false negative rate for first blood pressure value(a)130/80 mmHg for hypertension by simplified blood pressure measurement method were 95.96%,99.60%,0.40%,and 4.04%,the proportion of secondary blood pressure measurements was 27.27%,and the Kappa value with standard methods for screening hypertension was 0.959.Conclusion:In the simplified blood pressure measurement method,determining the secondary blood pressure measurement by the first blood pressure value(a)130/80 mmHg can ensure high consistency with the screening results of standard methods and significantly reduce the proportion of secondary blood pressure measurement,shorten the measurement time and improve efficiency,which is worth popularizing and application.
