摘　　要：目的探讨反复发作BPPV患者的可能病因及治疗体会。方法回顾分析2018年1月至2021年6月首诊我科反复发作的BPPV患者18人病因,讨论可能发病的机制及治疗体会。结果18例患者因不同原因所致反复发作性BPPV患者,经多次手法复位、前庭功能锻炼、药物治疗等综合治疗后17例患者随访半年后无再发,1例患者眩晕无改善。结论反复发作的BPPV复位效果差的可能原因有内耳疾病,内耳内环境的破坏及多管结石等。治疗上应以手法复位为主的非手术治疗效果良好。Objective To study the pathogenesis and treatment of intractable benign paroxysmal positional vertigo.Methods Clinical data of 18 patients with intractable BPPV were retrospectively reviewed for possible pathogenic factors and treatment.Results Treatment combining reposition maneuvers,vestibular rehabilitation training and medication resulted in symptom disappearance with no recurrence in 17 patients,1 patient still had vertigo.Conclusion It may be due to inner ear disease,such as trauma caused by damage to the inner ear and multi-tube for patients with intractable BPPV.It is good treatment based on reposition maneuvers of non-surgical.
