痛泻要方加味联合针灸治疗腹泻型肠易激综合征临床研究

摘　　要：目的:观察痛泻要方加味联合针灸治疗肝气乘脾证腹泻型肠易激综合征(IBS-D)的临床疗效。方法:选取80例肝气乘脾证IBS-D患者,按随机数字表法分为观察组及对照组各40例。观察组给予痛泻要方加味联合针灸治疗,对照组给予双歧杆菌三联活菌散(培菲康)口服治疗。2组均治疗4周。比较2组临床疗效,比较2组治疗前后粪便杆球菌比例、肠道双歧杆菌/大肠杆菌(B/E)比值的变化。结果:治疗4周后,观察组临床疗效总有效率为85.0%,对照组为67.5%,2组比较,差异有统计学意义(P<0.05)。治疗4周后,2组粪便杆球菌比例均较治疗前升高,观察组指标值高于对照组,差异均有统计学意义(P<0.05)。治疗4周后,2组B/E值均较治疗前升高,观察组B/E值高于对照组,差异均有统计学意义(P<0.05)。结论:痛泻要方加味联合针灸治疗肝气乘脾证IBS-D疗效较好,能调节肠道菌群,缓解临床症状,增加肠道有益菌。Objective: To observe the clinical effect of the therapy of modified Tongxie Yaofang combined with acupuncture and moxibustion on irritable bowel syndrome with diarrhea(IBS-D) with liver qi over-restricting spleen syndrome. Methods: A total of 80 IBS-D patients with liver qi over-restricting spleen syndrome were selected and divided into the observation group and the control group according to the random number table method,with 40 cases in each group. The observation group was treated with modified Tongxie Yaofang combined with acupuncture and moxibustion,and the control group was given live combined bifidobacterium,lactobacillus and enterococcus powder orally. Both groups were treated for four weeks. The clinical effects were compared between the two groups. Before and after treatment,the changes of proportion of bacteria in stool and ratio of bifidobacteria to escherichia coli(B/E) in intestine were compared between the two groups. Results:After four weeks of treatment,the total clinical effective rate was 85.0% in the observation group and 67.5% in the control group,the difference being significant(P<0.05). After four weeks of treatment,the proportion of bacteria in stool in the two groups was increased when compared with those before treatment, and the above index in the observation group was higher than that in the control group,differences being significant(P<0.05). After four weeks of treatment,the value of B/E in the two groups was increased when compared with that before treatment,and the above value in the observation group was higher than that in the control group,differences being significant(P<0.05). Conclusion:The therapy of modified Tongxie Yaofang combined with acupuncture and moxibustion has a good curative effect on IBS-D with liver qi over-restricting spleen syndrome,which can regulate the intestinal flora,relieve the clinical symptoms,and increase the intestinal available bacteria.
