补脾生肌益肺汤联合西药治疗老年慢性阻塞性肺疾病稳定期临床研究

摘　　要：目的:观察补脾生肌益肺汤联合西药治疗老年慢性阻塞性肺疾病(COPD)稳定期的临床疗效。方法:按照随机数字表法将116例老年COPD稳定期患者分为对照组与观察组各58例。对照组给予常规西药治疗,观察组在对照组基础上给予补脾生肌益肺汤治疗。比较2组治疗前后中医证候积分、用力肺活量(FVC)、第1秒用力呼气容积(FEV_(1))、第1秒用力呼气容积占用力肺活量百分比(FEV_(1)/FVC)、第1秒用力呼气容积占预测值百分比(FEV_(1)%)。比较2组不良反应发生情况。结果:与同组治疗前比较,治疗后2组喘息、咳嗽、咳痰、胸膈满闷、哮鸣音等中医证候积分均降低(P<0.05);与对照组治疗后比较,观察组治疗后以上各中医证候积分均较低(P<0.05)。与同组治疗前比较,治疗后2组FVC、FEV_(1)、FEV_(1)/FVC、FEV_(1)%均升高(P<0.05);与对照组治疗后比较,观察组治疗后FVC、FEV_(1)、FEV_(1)/FVC、FEV_(1)%均较高(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:相比于单纯西药治疗,西药联合补脾生肌益肺汤可加快老年COPD稳定期患者临床症状的缓解,改善肺功能,且安全性较高。Objective:To observe the clinical effect of the therapy of Bupi Shengji Yifei tang combined with western medicine on senile patients with chronic obstructive pulmonary disease(COPD) at stable stage.Methods:A total of 116 senile patients with COPD at stable stage were selected and divided into the control group and the control group according to the random number table method,with 58 cases in each group.The control group was treated with routine western medicine treatment,and the observation group was additionally treated with Bupi Shengji Yifei tang based on the treatment of the control group.Before and after treatment,the Chinese medicine syndrome scores,forced vital capacity(FVC),forced expiratory volume in one second(FEV_(1)),the ratio of forced expiratory volume in one second to forced vital capacity(FEV_(1)/FVC) and percentage of forced expiratory volume in one second(FEV_(1)%) were compared between the two groups.The incidence of adverse reactions was compared between the two groups.Results:After treatment,the Chinese medicine syndrome scores of wheezing,cough,expectoration,thoracic diaphragm distension and discomfort and wheezing rales in the two groups were decreased when compared with those in the same group before treatment(P<0.05);after treatment,the above Chinese medicine syndrome scores in the observation group were lower than those in the control group(P<0.05)After treatment,the levels of FVC,FEV_(1),FEV_(1)/FVC and FEV_(1)%in the two groups were increased when compared with those in the same group before treatment(P<0.05);after treatment,the above four levels in the observation group were higher than those in the control group(P<0.05).There was no significant difference being found in the comparison of incidence of adverse reactions between the two groups(P>0.05).Conclusion:Compared with simple western medicine,the therapy of western medicine combined with Bupi Shengji Yifei tang can accelerate the remission of clinical symptoms in senile patients with COPD at stable stage and improve the lung func
