温阳化瘀汤联合克林霉素磷酸酯片治疗慢性盆腔炎临床研究

摘　　要：目的:观察温阳化瘀汤联合克林霉素磷酸酯片治疗慢性盆腔炎的临床疗效及对炎症因子水平的影响。方法:将102例慢性盆腔炎患者按随机数字表法分为对照组和观察组各51例。对照组给予克林霉素磷酸酯片治疗,观察组在对照组基础上给予温阳化瘀汤治疗。比较2组治疗前后血清肿瘤坏死因子-α (TNF-α)、白细胞介素-6 (IL-6)、C-反应蛋白(CRP)、血液流变学指标及中医证候评分的变化,并评价2组临床疗效。结果:观察组临床总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,2组血清CRP、TNF-α及IL-6水平较治疗前降低(P<0.05),且观察组CRP、TNF-α及IL-6水平低于对照组(P<0.05)。治疗后,2组全血黏度、血浆黏度及纤维蛋白原(FIB)水平较治疗前降低(P<0.05),且观察组全血黏度、血浆黏度及FIB水平低于对照组(P<0.05)。治疗后,2组下腹痛、月经不调、白带异常及疲软无力等评分较治疗前降低(P<0.05),且观察组下腹痛、月经不调、白带异常及疲软无力等评分均低于对照组(P<0.05)。结论:温阳化瘀汤联合克林霉素磷酸酯片治疗慢性盆腔炎患者能够有效降低血清炎症因子水平,改善血液流变学指标,缓解临床症状,提高临床疗效。Objective:To observe the clinical effect of the therapy of Wenyang Huayu tang combined with clindamycin phosphate tablets on chronic pelvic inflammatory disease and its effect on levels of inflammatory factors. Methods: A total of 102 patients with chronic pelvic inflammatory disease were divided into the control group and the observation group according to the random number table method,with 51 cases in each group. The control group was treated with clindamycin phosphate tablets,and the observation group was additionally treated with Wenyang Huayu tang based on the treatment of the control group. Before and after treatment,the changes of serum tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),C-reactive protein(CRP),indexes of hemorheology and Chinese medicine syndrome scores were compared between the two groups. The clinical effects in the two groups were evaluated. Results:The clinical effective rate in the observation group was higher than that in the control group, the difference being significant(P<0.05). After treatment,the levels of serum CRP,TNF-α and IL-6 in the two groups were decreased when compared with those before treatment(P<0.05),and the above three levels in the observation group were lower than those in the observation group(P<0.05). After treatment,the levels of whole blood viscosity, plasma viscosity and fibrinogen(FIB) in the two groups were decreased when compared with those before treatment(P<0.05),and the above three levels in the observation group were lower than those in the control group(P<0.05). After treatment, the scores of lower abdominal pain,irregular menstruation, abnormal leucorrhea and fatigue and lack of strength in the two groups were decreased when compared with those before treatment(P<0.05), and the above four scores in the observation group were lower than those in the control group(P<0.05). Conclusion: The therapy of Wenyang Huayu tang combined with clindamycin phosphate tablets for patients with chronic pelvic inflammatory disease can effectively reduce the levels
