补肺汤合养心汤加减联合步行训练治疗慢性心力衰竭心肺气虚证临床研究

摘　　要：目的:观察补肺汤合养心汤加减联合步行训练治疗慢性心力衰竭心肺气虚证的临床疗效。方法:将60例慢性心力衰竭心肺气虚证患者以随机数字表法分为对照组、观察组,每组30例。2组均进行常规治疗,对照组在此基础上开展步行训练,观察组在对照组基础上给予补肺汤合养心汤加减治疗,2组均治疗4周。比较2组临床疗效、不良反应发生率,以及治疗前后中医证候积分、心功能指标。结果:观察组总有效率96.67%,高于对照组80.00%(P<0.05)。治疗后,2组中医证候积分均较治疗前降低(P<0.05),观察组中医证候积分低于对照组(P<0.05)。治疗后,2组脑钠肽(BNP)水平均较治疗前降低(P<0.05),6 min步行试验(6MWT)均较治疗前增大(P<0.05),左室射血分数(LVEF)均较治疗前升高(P<0.05);观察组BNP水平低于对照组(P<0.05),6MWT大于对照组(P<0.05),LVEF高于对照组(P<0.05)。观察组不良反应发生率10.00%,与对照组6.67%比较,差异无统计学意义(P>0.05)。结论:补肺汤合养心汤加减联合步行训练治疗可促进慢性心力衰竭心肺气虚证患者心功能康复,改善临床症状,安全性良好。Objective:To observe the clinical effect of modified Bufei tang and Yangxin tang combined with walking training for chronic heart failure with heart-lung qi deficiency syndrome.Methods:A total of60 cases of patients with chronic heart failure with heart-lung qi deficiency syndrome were divided into the control group and the observation group according to the random number table method,with 30 cases in each group.Both groups were given routine treatment,the control group was additionally given walking training based on the routine treatment,and the observation group was additionally treated with modified Bufei tang and Yangxin tang based on the treatment of the control group.Both groups were treated for four weeks.Chinese medicine syndrome scores and heart function indexes before and after treatment as well as the clinical effects and the incidence of adverse reactions in the two groups were compared.Results:The total effective rate was 96.67%in the observation group,higher than that of 80.00%in the control group(P<0.05).After treatment,Chinese medicine syndrome scores in the two groups were decreased when compared with those before treatment(P<0.05),and the score in the observation group was lower than that in the control group(P<0.05).After treatment,the levels of brain natriuretic peptide(BNP)in the two groups were decreased when compared with those before treatment(P<0.05),the six-minute walking test(6MWT)was increased(P<0.05),and the left ventricular ejection fraction(LVEF)was raised(P<0.05);the BNP level in the observation group was lower than that in the control group(P<0.05),6MWT was larger(P<0.05),and LVEF was higher(P<0.05).The incidence of adverse reactions was 10.00%in the observation group and 6.67%in the control group,there being no significance in the difference(P>0.05).Conclusion:The therapy of modified Bufei tang and Yangxin tang combined with walking training for patients with chronic heart failure with heart-lung qi deficiency syndrome can promote their rehabilitation of heart function and impro
