止喘汤联合茶碱缓释片治疗支气管哮喘急性发作期临床研究

摘　　要：目的:观察止喘汤联合茶碱缓释片治疗支气管哮喘急性发作期的临床疗效。方法:选取96例支气管哮喘急性发作期患者,采用随机数字表法分为对照组和研究组各48例。2组均给予常规对症治疗,对照组在此基础上给予茶碱缓释片治疗,研究组在对照组基础上给予止喘汤治疗,2组均治疗7 d。比较2组临床疗效、临床症状消失时间,以及治疗前后肺功能指标、免疫球蛋白E(IgE)水平和嗜酸性粒细胞(EOS)计数。结果:研究组总有效率95.83%,高于对照组83.33%(P<0.05)。研究组咳嗽、喘息、咳痰及哮鸣音消失时间均短于对照组(P<0.05)。治疗后,2组用力肺活量(FVC)、第1秒用力呼气容积(FEV_(1))及最大呼气流量(PEF)均较治疗前增大(P<0.05),研究组FVC、FEV_(1)、PEF均大于对照组(P<0.05)。治疗后,2组IgE水平及EOS计数均较治疗前降低(P<0.05),研究组IgE水平及EOS计数均低于对照组(P<0.05)。结论:止喘汤联合茶碱缓释片治疗支气管哮喘急性发作期疗效确切,可促进临床症状缓解,改善肺功能,减轻气道炎症反应。Objective:To observe the clinical effect of the therapy of Zhichuan tang combined with theophylline sustained-release tablets on acute exacerbation of bronchial asthma.Methods:A total of96 patients with acute exacerbation of bronchial asthma were selected and assigned to the control group and the study group according to the random number table method,with 48 cases in each group.Both groups were treated with routine symptomatic treatment.The control group was additionally treated with theophylline sustained-release tablets,and the study group was additionally treated with Zhichuan tang based on the treatment of the control group.Both groups were treated for seven days.The indexes of lung function,levels of immunoglobulin E(IgE)and eosinophils(EOS)count before and after treatment,and clinical effects and disappearance time of clinical symptoms were compared between the two groups.Results:The total effective rate was 95.83%in the study group,higher than that of 83.33%in the control group(P<0.05).The disappearance time of cough,gasping,expectoration and wheezing rales in the study group were shorter than those in the control group(P<0.05).After treatment,the forced vital capacity(FVC),forced expiratory volume in one second(FEV_(1))and peak expiratory flow(PEF)in the two groups were increased when compared with those before treatment(P<0.05),and the FVC,FEV_(1) and PEF in the study group were higher than those in the control group(P<0.05).After treatment,the levels of IgE and EOS count in the two groups were decreased when compared with those before treatment(P<0.05),and the level of IgE and EOS count in the study group were lower than those in the control group(P<0.05).Conclusion:The therapy of Zhichuan tang combined with theophylline sustained-release tablets has a definite curative effect on acute exacerbation of bronchial asthma,which can promote the remission of clinical symptoms,improve the lung function,and reduce the airway inflammation.
