桔贝合剂联合甲泼尼龙治疗重症肺炎临床研究

摘　　要：目的:观察桔贝合剂联合甲泼尼龙治疗重症肺炎的临床疗效,以及对患者T淋巴细胞亚群、血氧分压和氧合指数的影响。方法:选取100例重症肺炎患者,根据随机数字表法分为对照组与治疗组各50例。对照组给予注射用甲泼尼龙琥珀酸钠治疗,治疗组在对照组基础上给予桔贝合剂治疗,2组均治疗14 d。比较2组临床疗效、症状改善时间、住院时间,以及治疗前后T淋巴细胞亚群、血氧分压和氧合指数。结果:治疗组总有效率96.00%,高于对照组78.00%(P<0.05)。治疗组退热、止咳、肺部阴影消失时间及住院时间均短于对照组(P<0.05)。治疗后,2组血氧分压、氧合指数均较治疗前升高(P<0.05),治疗组血氧分压、氧合指数均高于对照组(P<0.05)。治疗后,2组CD3^(+)、CD4^(+)水平及CD4^(+)/CD8^(+)值均较治疗前升高(P<0.05),CD8^(+)水平均较治疗前降低(P<0.05);治疗组CD3^(+)、CD4^(+)水平及CD4^(+)/CD8^(+)值均高于对照组(P<0.05),CD8^(+)水平低于对照组(P<0.05)。结论:桔贝合剂联合甲泼尼龙治疗重症肺炎,可有效改善患者的临床症状,提升呼吸和免疫功能。Objective:To observe the clinical effect of Jiebei mixture combined with methylprednisolone for severe pneumonia,and its effect on T lymphocyte subsets,partial pressure of blood oxygen and oxygenation indexes.Methods:A total of 100 cases of patients with severe pneumonia were selected and divided into the control group and the treatment group according to the random number table method,with50 cases in each group.The control group was treated with methylprednisolone sodium succinate for injection,and the treatment group was additionally treated with Jiebei mixture based on the treatment of the control group.Both groups were treated for fourteen days.The T lymphocyte subsets,partial pressure of blood oxygen and oxygenation indexes before and after treatment as well as the clinical effects,the improvement time for symptoms and the hospitalization time in the two groups were compared.Results:The total effective rate was 96.00%in the treatment group,higher than that of 78.00%in the control group(P<0.05).The time of defervescence,relieving cough and disappearance of lung shadow as well as the hospitalization time in the treatment group were shorter than those in the control group(P<0.05).After treatment,the partial pressure of blood oxygen and oxygenation indexes in the two groups were increased when compared with those before treatment(P<0.05),and the blood oxygen and oxygenation indexes in the treatment group were higher than those in the control group(P<0.05).After treatment,the levels of CD3^(+)and CD4^(+)and the values of CD4^(+)/CD8^(+)in the two groups were increased when compared with those before treatment(P<0.05),and the CD8^(+)level was decreased(P<0.05);the levels of CD3^(+)and CD4^(+)and the value of CD4^(+)/CD8^(+)in the treatment group were higher than those in the control group(P<0.05),and the CD8^(+)level was lower(P<0.05).Conclusion:Jiebei mixture combined with methylprednisolone for severe pneumonia can effectively improve clinical symptoms,respiration and immune function.
