摘 要:目的 探讨全身亚低温治疗对中、重度新生儿缺氧缺血性脑病(HIE)早期振幅整合脑电图(aEEG)及血清神经元特异性烯醇化酶(NSE)的影响。方法 选择2019年1月~2020年10月潍坊市妇幼保健院新生儿科收治的中、重度HIE患儿40例,分为对照组20例和实验组20例。对照组给予常规对症支持治疗,实验组在常规治疗基础上实施全身亚低温治疗。比较治疗前后两组患儿aEEG、NSE的变化。结果 治疗前实验组和对照组aEEG及NSE无明显差异(P>0.05)。治疗后24h、72h实验组aEEG的睡眠觉醒周期及aEEG评分总分数值优于对照组,实验组血清NSE的72h含量低于对照组(P<0.05)。结论 早期应用亚低温是治疗新生儿缺氧缺血性脑病的有效手段之一,可保护患儿脑功能,改善预后。Objective To explore the effect of systemic mild hypothermia treatment on the early amplitude integrated electroencephalogram(aEEG) and serum neuron-specific enolase(NSE) of moderate and severe neonatal hypoxic-ischemic encephalopathy(HIE).Methods From January 2019 to October 2020,forty children with moderate and severe HIE admitted to the Department of Neonatology, Weifang Maternal and Child Health Hospital were selected and divided into control group 20 cases and experimental group 20 cases.The control group was given routine symptomatic and supportive treatment, and the experimental group was given systemic hypothermia treatment on the basis of routine treatment.Comparison was made in amplitude integrated EEG and serum neuron-specific enolase between the two groups of children before and after treatment.Results There was no significant difference in aEEG and NSE between the experimental group and the control group before treatment(P>0.05).Twenty-four hours and seventy-two hours after treatment, the sleep-wake cycle and aEEG score of the experimental group were better than those of the control group, and the 72 h serum NSE level of the experimental group was lower than that of the control group.Conclusion Early application of mild hypothermia is one of the effective methods for the treatment of neonatal hypoxic-ischemic encephalopathy, protecting the brain function of children and improving the prognosis of children.
