内镜逆行胰胆管造影术治疗胆囊管结石合并胆总管结石7例分析

摘　　要：目的 评估内镜逆行胰胆管造影术治疗胆囊管结石合并胆总管结石治疗效果。方法 选取威海市立医院2020年1月~2022年1月收治的胆囊管结石合并胆总管结石的患者7例,采取内镜逆行胰胆管造影术治疗胆囊管结石合并胆总管结石。结果 7例患者均成功完成选择性胆管造影,均成功行胆总管取石。超选胆囊管顺利显影5例(71.4%),4例(80%)成功取出胆囊管结石,1例结石过大未能顺利取出,将其送入胆囊,未发生出血、穿孔、胆汁漏、死亡等严重并发症。结论 对于胆囊管结石合并胆总管结石,可考虑在ERCP术中取出胆总管结石的同时,超选择胆囊管后取石。Objective To evaluate the efficacy of endoscopic retrograde cholangiopancreatography(ERCP) in the treatment of cystic duct stones combined with common bile duct stones.Methods Seven patients with cystic duct stones combined with common bile duct stones were treated by endoscopic retrograde cholangiopancreatography(ERCP) from January 2020 to January 2022 in Weihai Municipal Hospital.Results Selective cholangiography and choledocholithotomy were successfully performed in all 7 patients.Superselective cystic duct opacification was successful in 5 cases(71.4%),without any serious complications such as bleeding, perforation, bile fistula, death and so on.Four cases of cystic duct stones were successfully removed, and 1 case of stone was too large to be removed, thus it was sent into the gallbladder.Conclusions For patients with cystic duct stones combined with common bile duct stones, it can be considered to take out cystic duct stones after superselective choledocholithotomy while taking out common bile duct stones during ERCP.
