垂直切口乳房缩小术后瘢痕增生的防治策略

摘　　要：目的探讨减少垂直瘢痕乳房缩小手术患者术后瘢痕的方法。方法本研究系统回顾了2020年1月至2021年6月在本中心接受垂直瘢痕乳房缩小术的23名患者治疗过程及术后瘢痕形态。所有患者术前设计遵循缩短切口及减小猫耳畸形的原则、术中采用无创操作及减张缝合、术后接受系统性抗瘢痕治疗。结果患者术后随访12-18个月,乳房瘢痕均无明显发红、隆起、增宽等,乳房形态良好自然,患者对手术效果满意。结论本研究结果证实,通过在术前设计、术中操作及术后处理三方面遵循减少瘢痕处理原则,可有效减轻垂直瘢痕乳房缩小手术术后瘢痕,提高手术效果。Objective Explore the methods to reduce postoperative scar in patients receiving vertical scar reduction mammaplasty.Methods From January 2020 to June 2021,medical history from 23 patients who underwent vertical scar reduction mammaplasty were retrospectively analyzed.Minimizing scar and dog ear deformity was main principle followed in preopertaive design,non-invasive technique and tension reduction suture were two key intraoperative techniques,systematic anti-scar treatment were used in postoperative follow up.Results Patients were followed up for 12 to 18 months after the operation.There was no redness,bulge or widening of vertical scar,breast shapes were natural,patients were satisfied with the surgical results.Conclusions This study revealed that minor scarring can be achieved through appropriate preoperative design,no-invasive and tension reduction intraoperative technique,postperative anti-scar treatment.
