脂肪抽吸治疗RohrichⅠa、Ⅱa级男性乳房肥大症疗效分析

摘　　要：目的探讨脂肪抽吸治疗RohrichⅠa级和Ⅱa级[1]男性乳房肥大症疗效及美容效果。方法自2020年10月至2022年8月收治的19例RohrichⅠa级和Ⅱa级男性乳房肥大症患者,在乳房下皱襞取0.3~0.5cm切口,采用负压抽吸该区域内的脂肪组织。结果19例患者单侧手术时间50~80min,出血量30~50ml,共吸出脂肪量500~800ml,术后住院时间3-5d。切口隐蔽,均一期愈合的;术后无感染、乳头乳晕坏死、血肿等并发症。10例患者随访3个月,患者乳房形态自然,无明显瘢痕,满意度可。结论脂肪抽吸术治疗RohrichⅠa级和Ⅱa级男性乳房肥大通过残留较小的术后瘢痕从而对肥大乳房重新塑形,具有操作简单方便、恢复较快、损伤轻、出血少、瘢痕隐蔽等优点,适用于治疗脂肪型及脂肪腺体混合型的轻中度男性乳房肥大症。Objective To investigate the efficacy and cosmetic results of liposuction for the treatment of Rohrich grade Ⅰa and Ⅱa gynecomastia. Methods Nineteen patients with Rohrich grade Ⅰa and Ⅱa gynecomastia admitted from October 2020 to August 2022 were treated with a 0.3-0.5 cm incision in the inframammary fold, and fat tissue in the area was aspirated using negative pressure. Results Nineteen patients had a unilateral operative time of 50-80 min, bleeding of 30-50 ml, and a total of 500-800 ml of fat aspirated. The incisions were hidden and all healed in one stage;there were no complications such as infection, nipple areola necrosis and hematoma after surgery. 10 patients were followed up for 3 months, and the patients had natural breast shape, no obvious scar and satisfaction. Conclusion Liposuction for Rohrich grade Ⅰa and Ⅱa gynecomastia is suitable for the treatment of mild to moderate adipose and mixed adipose glandular gynecomastia by leaving a smaller postoperative scar and thus reshaping the hypertrophied breast, which has the advantages of simple and convenient operation, faster recovery, light injury, less bleeding, and concealed scar.
