摘　　要：目的观察局部注射脂肪细胞外基质凝胶(SVF-gel)治疗增生性瘢痕的临床疗效,为临床治疗增生性瘢痕探索新的途径。方法2019年6月~2022年5月于郑州市第一人民医院整形科门诊就诊的面部增生性瘢痕患者12人,采用Coleman技术获得其下腹部的颗粒脂肪,进一步通过物理方法获得脂肪萃取物SVF-gel,将其精细化注射于瘢痕部位皮下浅层,通过电话和门诊随访,分别在术前、治疗后6个月时拍摄瘢痕区域照片,此外,使用患者和观察者瘢痕评价量表(patient and observer scar assessment scale,POSAS)评估治疗前后瘢痕情况,同时进行患者满意度调查,评价整体治疗效果,进行统计学分析。结果术后6个月,12例患者注射区域的瘢痕质地均较术前有明显改善,未出现明显手术并发症,瘢痕质地均逐渐趋近于正常皮肤,POSAS评分在色泽、柔韧度、厚度,瘙痒程度等方面评分及总分均明显低于术前(P<0.01),12例患者治疗满意度平均分4.5分(0-5分).结论SVF-gel可用于瘢痕部位的精细化注射,能够明显改善瘢痕质地、弹性,软化瘢痕,促进增生性瘢痕重塑。Objective To observe the clinical effect of local injection of adipose extracellular matrix vascular fraction gel(SVFgel)in the treatment of hypertrophic scar and to explore new ways for the treatment.Methods From June 2019 to May 2022,twelve patients with facial hypertrophic scar who attended the outpatient Department of Plastic Surgery of the First People's Hospital of Zhengzhou City were treated with the Coleman technique to obtain granular fat from their lower abdomen,and further obtained SVF-gel by physical methods,which was finely injected into the superficial subcutaneous layer of the scar site,and followed up by telephone and outpatient visits,respectively,at preoperative In addition,the patient and observer scar assessment scale(POSAS)was used to assess the scar condition before and after the treatment,and a patient satisfaction survey was also conducted to evaluate the overall treatment effect and to perform statistical analysis.Results Six months after surgery,the scar texture of the injected area in all 12 patients was significantly improved compared with that before surgery,no significant surgical complications occurred,and the scar texture gradually approximated to normal skin.Conclusion SVF-gel can be used for refinement of scar site injection,which can significantly improve scar texture and elasticity,soften scar and promote remodeling of hyperplastic scar.
