摘　　要：目的探讨同时达到阴道腔缩窄和阴道壁饱满效果的复合式阴道紧缩术式及其临床效果。方法从作者手术项目组2021年1月-2021年12月期间实施的复合式阴道紧缩术病例中,随机选取50例作为观察对象(N=50);均按照“改良型阶梯式倒U型”手术设计,首先采用脱细胞异体真皮(ADM)进行阴道腔缩窄,然后制作ADM颗粒进行阴道壁填充;术后即刻观察阴道腔缩窄及阴道壁饱满情况,并计算平均手术时间,术后3个月随访求美者对手术效果(阴道口闭合情况、阴道控制力及包裹感等)的满意度。结果①50例复合式阴道紧缩术平均手术时间为(1.0±0.2)h。②术后即刻阴道腔缩窄有效率及阴道壁饱满有效率均为100%。③术后3个月求美者对阴道口闭合情况、阴道控制力及包裹感等满意度达94%。结论使用ADM缩窄阴道腔及其颗粒填充阴道壁是一种有效的复合式阴道紧缩术式,可同时达到阴道紧缩和阴道壁饱满的效果,有利于提高阴道的控制力和包裹感,具有临床可行性。Objective To explore a new compound vaginal tightening technique and its clinical effect through narrowing the vaginal cavity and filling the vaginal wall simultaneously.Methods Fifty patients(N=50)who has undergone compound vaginal tightening operation from January 2021 to December 2021 were randomly selected.Firstly,the vaginal cavity was constricted with the band of Acellular Dermal Matrix(ADM)designed by the“improved stepped inverting U-shape”,then the vaginal wall was filled by particles made of the rest of ADM band before.Finally,average operation time was calculated,the the vaginal cavity narrowed and vaginal wall thickened were observed,the patients satisfaction rate to such as the closure of vaginal orifice,the control force of vagina and the sense of wrapping were followed up 3 months after operation.Results①The average time of 50 operation was 1.0±0.2 hours.②The effective rate of vaginal cavity narrowed and vaginal wall thickened immediately was 100%.③There were 94%of the patients satisfied with the closure of vaginal orifice,vaginal control and sense of wrapping 3 months after operation.Conclusion Using ADM band to narrow the vaginal cavity and its particles to fill the vaginal wall is an effective compound vaginal tightening technique,which could promote the vaginal control and the sense of wrapping by vaginal tightening and vaginal wall fullness.It suggested that the technique could be applied to clinic.
