毫针联合火针密刺治疗慢性湿疹脾虚湿蕴证的临床研究

摘　　要：目的:观察毫针联合火针密刺法治疗慢性湿疹脾虚湿蕴证的临床疗效。方法:将64例慢性湿疹脾虚湿蕴证患者随机分为试验组和对照组各32例。试验组采用毫针联合火针密刺法,针刺治疗隔日1次,每周3次,共4周;对照组使用糠酸莫米松乳膏外涂治疗,1次/d,共4周。分别于治疗前、治疗4周后及治疗结束4周后随访时比较两组患者的湿疹面积及严重度指数(EASI)评分、瘙痒程度评分及中医证候评分,并评定疗效。结果:两组各完成30例。试验组总有效率为93.33%(28/30),对照组总有效率为73.33%(22/30);试验组疗效优于对照组,差异有统计学意义(P<0.05)。治疗后及随访时,两组患者EASI评分、瘙痒程度评分、中医证候评分均较治疗前降低(P<0.05);试验组患者治疗后瘙痒程度评分、中医证候评分均低于对照组(P<0.05)。结论:毫针联合火针密刺法治疗慢性湿疹脾虚湿蕴证具有显著临床疗效。Objective:To observe the clinical effect of filiform acupuncture combined with dense needling of fire acupuncture on chronic eczema of spleen deficiency with dampness accumulation syndrome.Methods:Totally 64 patients with chronic eczema of spleen deficiency with dampness accumulation syndrome were randomly divided into treatment and control groups,32 cases in each group.Patients were treated with filiform needle combined with dense needling of fire acupuncture in treatment group.Acupuncture treatment was performed once every other day,3 times a week for 4 weeks.Patients were treated with mometasone furoate cream in control group,once a day for 4 weeks.The eczema area and severity index(EASI),pruritus degree and traditional Chinese medicine(TCM)syndrome scores were evaluated and compared between the two groups before treatment,after treatment of 4 weeks and during follow-up 4 weeks after treatment,and the clinical curative effect was evaluated finally.Results:There were 30 cases completed respectively in each group in the end.The total effective rate was 93.33%(28/30)in the treatment group and 73.33%(22/30)in control group.The treatment group showed better clinical curative effect than control group and the difference was statistically significant(P<0.05).After treatment and during follow-up,the EASI score,pruritus score and TCM syndrome score decreased in two groups(P<0.05).The treatment group showed lower pruritus score and TCM syndrome score than control group(P<0.05).Conclusion:Filiform acupuncture combined with dense needling of fire acupuncture show a significant clinical effect in the treatment of chronic eczema of spleen deficiency with dampness accumulation syndrome.
