通络宁血方治疗过敏性紫癜肾炎的临床研究

摘　　要：目的:探讨通络宁血方治疗过敏性紫癜肾炎(HSPN)血热伤络证的效果。方法:采用随机数字表法将80例HSPN血热伤络证患者分为治疗组和对照组,每组40例。治疗组患者采用通络宁血方联合常规西医治疗,对照组患者采用常规西医治疗,两组均治疗3个月。比较两组患者治疗前、治疗3个月后尿红细胞计数、24 h尿蛋白定量(24UP)、血清肌酐(Scr)、免疫球蛋白(IgA、IgM、IgG)及不良反应(胃肠道症状、肝功能异常、头晕)发生率。结果:治疗组总有效率为95.00%(38/40),对照组总有效率为77.50%(31/40),治疗组高于对照组,差异有统计学意义(P<0.05);治疗3个月后,两组患者中医证候积分、尿红细胞计数、24UP、Scr、IgA、IgM均较治疗前降低,且治疗组患者中医证候积分、尿红细胞计数、24UP、Scr、IgA、IgM低于对照组,差异均有统计学意义(P<0.05);治疗3个月后,两组患者IgG均较治疗前升高,且治疗组患者IgG高于对照组,差异均有统计学意义(P<0.05);治疗3个月后,两组患者肝功能指标、胃肠道症状发生率、皮疹发生率、头晕发生率比较,差异均无统计学意义(P>0.05)。结论:通络宁血方治疗HSPN血热伤络证具有较好疗效,可改善患者肾功能,纠正免疫紊乱,且不会增加不良反应。Objective:To explore the effect of Tongluo Ningxue decoction in the treatment of allergic purpura nephritis(HSPN)with Xuere Shangluo syndrome(血热伤络证).Methods:According to random number table method,80 HSPN patients with Xuere Shangluo syndrome were divided into treatment and control groups,40 cases in each group.The treatment group was treated by Tongluo Ningxue decoction combined with conventional western medicine treatment,while the control group was given conventional western medicine treatment,and both groups were treated for 3 months.The urine red blood cell count,24 h urine protein quantitative(24UP),serum creatinine(Scr),immunoglobulin(IgA,IgM and IgG)and adverse reactions(gastrointestinal symptoms,liver dysfunction and dizziness)were compared between the two groups before and after treatment of 3 months.Results:The total effective rate was 95.00%(38/40)in treatment group and 77.50%(31/40)in control group.The treatment group showed higher total effective rate than control group and the difference was statistically significant(P<0.05).After 3 months of treatment,the TCM syndrome scores,urine red blood cell count,24UP,Scr,IgA and IgM decreased in the two groups,and the treatment group showed lower TCM syndrome scores,urine red blood cell count,24UP,Scr,IgA and IgM than control group,with statistically significant difference(P<0.05).After 3 months of treatment,the IgG increased in the two groups and the treatment group showed higher IgG than control group,with statistically significant differences(P<0.05).After 3 months of treatment,there were no significant differences in liver function indexes,incidence of gastrointestinal symptoms,incidence of rash and incidence of dizziness between the two groups(P>0.05).Conclusion:Tongluo Ningxue decoction shows good curative effect in the treatment of HSPN with Xuere Shangluo syndrome.It can improve the renal function of patients and correct immune disorders,and it will not increase the occurrence of adverse reactions.
